DUODENAL SWITCH (A PUNCH AGAINST OBESITY)

REVISION OF Roux en Y GASTRIC BYPASS
When patients who had weight loss surgery in the past require other procedures to correct the complications or undo the ill effects of their original operation, it is called a revisional weight loss surgical procedure.The clients who seek revisional surgery may have lost their weight only to gain it back or some may have had inadequate weight loss. There are also those patients that had ill effects of their primary operations including ulceration and stricture, in the case of Roux en Y gastric bypass, and slippage and erosion , in case of adjustable gastric banding.There are some patients who have been able to lose the weight and keep it off; however, this comes at the cost of a near constant nausea and frequent vomiting.The failure of the primary surgery is frequently blamed on the patient, however that is usually not the case. A less than ideal outcome of a weight loss surgical procedure can be traced back to a procedure that did not work for that particular patient. Its just like trying a number of blood pressure medications to find the one that works best for them. Alternatively, the procedure may have delivered the best outcome possible which may be inadequate for that particular patient or the condition. Conditions that require revisional weight loss surgery include;
1) Weight regain after initial weight loss.
2.) Inadequate weight loss.

In almost all the cases the best option for Roux en Y gastric bypass that is in need of revision is the duodenal switch operation. Adjustable gastric banding ( Lap Band) placement as a weight loss surgery for a primary Roux en Y may only be indicated for patients that have had initial success of weight loss followed by weight regain. This however, should be in the absence of dumping syndrome, marginal ulcers or reflux disease, which can potentially get exacerbated by placement of a band on top of the gastric pouch.
Adjusting the length of the common channel, alimentary limb, allows a revisional weight loss surgery to be tailored to the patient's needs. In case of a patient having a revision of Roux en Y for persistent nausea and vomiting with an adequate weight loss, a relatively long common channel and alimentary limb ( percentage based ) will be set for the patient, thus preventing any further weight loss yet correcting the persistent nausea and vomiting issue. In contrast, a patient that is seeking revision of a failed gastric bypass to duodenal switch for inadequate weight loss or weight gain will have a relatively shorter common alimentary channel (percentage based) in order to maximise the amount of weight loss. Revising a failed gastric bypass from proximal to distal Roux en Y is seen to be a poor choice in the majority of patients due to the fact that the distal gastric bypass has the worst nutritional safety profile of all the known surgical procedures. Consult your bariatric surgeon for any complications post gastric bypass. This can be corrected by a revisional surgery.

Laceration Repair (Cuts and grazes)
Introduction
Cuts and grazes are a common type of injury and, in most cases, do not pose a threat to health.
Treating minor cuts and grazes
Most cuts and grazes are minor and can be easily treated at home.
Cleaning them thoroughly and covering them with a plaster or dressing is all that is needed.
Stopping the bleeding
If your cut or graze is bleeding heavily or is on a particularly delicate area of your body, such as the palm of your hand, you should stop the bleeding before applying any kind of dressing.
Apply pressure to the area using a bandage or a towel. If the cut is to your hand or arm, raise it above your head. If the injury is to a lower limb, lie down and raise the affected area above the level of your heart so the bleeding slows down and stops.
Dressing
To dress a cut or graze at home:
Wash and dry your hands thoroughly
Clean the wound under running tap water but do not use antiseptic because it may damage the tissue and slow down healing
Pat the area dry with a clean towel
Apply a sterile, adhesive dressing, such as a plaster
Keep the dressing clean by changing it as often as necessary and keep the wound dry by using waterproof dressings, which will allow you to take showers.
Painkillers
The wound should heal by itself in a few days. If the wound is painful, you can take painkillers, such as paracetamol or ibuprofen.
When to see Dr. B C Shah
You only need to see Dr. B C Shah if there is a risk of a cut or graze becoming infected or you think it already has become infected.
You are more at risk of the wound becoming infected if:
It has been contaminated with soil, pus, bodily fluids or faeces (stools)
There was something in the wound before it was cleaned, such as a tooth or a shard of glass
The wound has a jagged edge
The wound is longer than 5cm (1.9 inches)
You should also contact Dr. B C Shah if your skin has been bitten (either by an animal or a person), as bites are prone to infection.
Signs that a wound has become infected include:
Swelling of the affected area
Pus forming in the affected area
Redness spreading from the cut or graze
Increasing pain in the wound
Feeling generally unwell
A high temperature (fever) of 38°C (100.4°F) or above
Swollen glands
An infected wound can usually be successfully treated with a short course of antibiotics (usually around seven days).
When to seek emergency help
Some cuts and grazes can be more serious and will require an emergency treatment.
It is recommended that you visit Dr. B C Shah if:
You are bleeding from a cut artery. Blood from an artery comes out in spurts (with each beat of the heart), is bright red and is usually hard to control.
You cannot stop the bleeding.
You experience loss of sensation near the wound or you are having trouble moving body parts. If this is the case you may have damaged underlying nerves.
There is severe pain, extensive bruising and you are having trouble moving body parts. If this is the case you may have damaged one of your tendons.
You have received a cut to the face. You may require urgent treatment to prevent scarring.
You have received a cut to the palm of your hand and the cut looks infected. These types of infection can spread quickly.
There is a possibility that a foreign body is still inside the wound.
The cut is extensive, complex or has caused a lot of tissue damage.
Your cut will be examined to determine whether or not there is any risk of infection. If there was glass inside your cut, you may need an X-ray to ensure it has been removed.
What happens if there is no risk of infection
If there is no risk of infection, your cut will be cleaned using water or a sterile saline solution before it’s closed. This may be done using stitches, tissue adhesive or skin-closure strips.
Stitches (sutures). These are usually used to close cuts that are more than 5cm long, or wounds that are particularly deep. A sterile surgical thread is used for stitches, which is flexible and allows the wound to move.
Tissue adhesive (glue). This may be used to close less severe cuts that are less than 5cm long. The tissue adhesive is painted onto your skin, over your cut, while the edges are held together. The paste then dries, forming a flexible layer that keeps the cut closed.
Skin-closure strips. These may be used as an alternative to tissue adhesive, for cuts that are less than 5cm long, where there is a risk of infection. The strips are sticky and can be placed over the edges of the cut to hold them together. They are easier to remove than tissue adhesive.
Once your cut is closed, it may be covered with a protective dressing to ensure that your stitches, tissue adhesive or skin-closure strips stay in place.
If you have stitches or strips, you will need to return to Dr. B C Shah to have them removed.
Stitches or strips on the head are removed after three to five days
Stitches over joints are removed after 10-14 days
Stitches or strips at other sites are removed after 7-10 days
You should never try to remove stitches yourself. They should only be removed by Dr. B C Shah
.
Tissue adhesive comes off by itself after a week or so.
To prevent tetanus (a serious bacterial infection), you may be given a tetanus booster injection. If it’s suspected that you are at risk of developing tetanus, you may be referred for specialist treatment.
If there is risk of infection, or your cut is already infected
If there is risk of infection or your cut is already infected, Dr. B C Shah may take a sample for analysis using a swab, before cleaning it as usual.
However, they will not yet be able to close your cut because this may trap any infection inside it, making it more likely to spread. Instead, the cut will be packed with a non-sticky dressing so that it cannot close, before it is covered with a protective dressing. You may also be given antibiotics to fight the infection.
You will need to return Dr. B C Shah after three to five days to see if any infection has cleared up. If so, your cut will be closed using stitches or skin-closure strips.
If your infection has not cleared up, a change of antibiotics may be required.
Skin grafts
If your graze is very severe and you have lost a lot of skin, you may need to have a skin graft.
Dr. B C Shah will take some skin from another part of your body and put it over the wound. After a while, it will heal and look normal.

Breast infection in infants
Baby L_____ S_____, one month old female infant was brought to me by her aunt with a huge left breast abscess. The mother was too terrified to come inside my clinic & the father was also outside consoling the mother. It started as a small boil. They had taken her to pediatrician who gave them local ointment and oral antibiotics. However, it did not subside. IT grew very quickly and the child started to have fever. It was extremely painful and the child was constantly crying. The baby was being massaged by a servant. The possible reason is that many times such masseur, who are most often untrained try manipulation the delicate neonatal breast to express a clear or cloudy (milk-like) substance from nipple (also called witch's milk). This causes potential damage & bleeding inside which gets infected or possibly it causes cracks in the nipple thro which the bacteria enter inside. The main worry of the relatives & parents was – will she have a normal breast development or is her left breast completely destroyed?
I removed the pus from the infant baby's left breast under anesthesia. Subsequently she underwent daily dressings. The nipple restored back to normal and the wound healed very soon. I assured the parents that she will have normal breast. Nature is great!
Adult breast is common especially in lactating mothers but neonatal breast abscess is not so common.

Miraculous healing from Burn Injuries

Hair transplantation: one of the solutions for hair loss
“Baldness” is the term used to describe the state of lack of hair on the head and androgenetic alopecia is the commonest cause of baldness. Approximately 50% of men are affected by the age of 50 years. One of the hallmarks of androgenetic alopecia is the conversion of thick terminal hairs into miniaturized; extremely thin (vellus) hairs. This process of miniaturization (i.e., shrinking of hair volume and growth length) is usually an indication that one is undergoing hair loss. The main mechanism for miniaturization relates to the shortening of the growth phase, i.e., the hair becomes “lazy” and does not grow to its full term. It typically begins in the frontal scalp and the vertex (crown area).
A significant proportion of women also suffer from hair loss. Unlike men, hair loss in females does not have any social acceptance. The chief complaint of women with female pattern hair loss (FPHL) is the ‘see through’ appearance of frontal hairline. These women suffer in silence for years together. The causes of hair loss in females can be: Female patterned hair loss (FPHL), telogen effluvium, hormonal imbalance, poor diet, diffuse alopecia areata. The progression hair loss in females (FPHL) is different from that of men. They experience diffuse thinning with no bald regions. Their frontal hairlines are relatively stable.

Hair transplantation is a cosmetic surgery which provides extra hair ranging in number from 1000 to 5000 to the bald scalp. The hairs along with the roots (follicular units) are taken from the back of scalp and are transplanted on to bald area on the frontal scalp and the vertex. The modern methods of hair transplantation involve transplantation of individual follicular units and provide completely natural appearance. These hairs remain life-long. They grow and undergo cutting just like natural hair. However, the progression of baldness is not arrested after surgery and the transplanted hair grow after 3-4 months. A combination of medicines and surgery thus provides the best final result. There are two methods of surgery:

Follicular unit transplantation (FUT): A horizontal strip, 1-2 cm wide and 10-20 cm long is harvested from the back of head, and dissected under stereomicroscopes to yield follicular units. It leaves a single linear scar hidden by the hair at the back of head.
Follicular unit extraction (FUE): The individual follicular units are harvested from the back and are then transplanted on to the frontal bald area. This propels the field of HT surgery one step closer to the elite minimally invasive status.
