NO PURELY GENERIC MEDICINE IN  MARKET HOW DOCTORS CAN WRITE ?

PROF .DRRAM,HIV /AIDS,HEPATITIS ,SEX DISEASES & WEAKNESS expert,New Delhi,India, profdrram@gmail.com,+917838059592,+919832025033,ON WHATSAPP

There is a hugh confusion in the mind of Doctors for Generic Medicines as MCI insist them to write only Generic Name of Medicines in their prescription to patients, many Doctors's Associations Like IMA,ASI,API etc also advise Doctors to follow it.As Pri Minister spoke clearly that Treatment is very costly in India as price of Branded medicines, stents and prostheses is very high,so it is policy of Government or Health Ministry & Drug Controller,Pharma Price Regulation Authority  to reduce price of medicines gradually by fixing price of medicines and stents. But are they are doing really good for patients in name of Generic medicines or simply blaming Doctors for making headlines as populist public alluring excercise.

Instead of targetting Doctors only for this, nothing concrete "Generic Medicines are available in India. In Government word Generic means the chemical name of a medicine as "Paracetamol or Omperazole. Surprisingly  nothing is available in Indian Market with this name only.Any new medicines if it is marketed then Pharma Companies sold it their Brand or Commercial name as Paracetamol sold in name of Calpol,Delo, Crocin etc,Every company use its own brand name so that they can market it and earn profit as Doctor will write medicine with these name (as these are promoted to doctors by medical representative of that company providing free samples, gifts, frebbies or sponsoring Doctor's scientific meet, annual CMEs in 3 to 5 star hotels or free air travel and stay etc) and Pharmacist will be bound to give that brand only.

Pharma Companies do write chemical name of these medicines in small letter too beneath Branded but Pharmacist give and  Patient takes only the brand written by Doctor as if Brand changed, many Doctors /Pharmacist object and tell patient clearly as if you take another brand ,quality may suffer and disease may not be cured. The price of different Brand vary from Pharma Company to pharma company as For example, amlodipine in India is still available as Amlogard (Pfizer) @ Rs. 8/-; however, Dr. Reddy’s Lab also markets it as Stamlo, at less than Re.1/-. So a profit of 100 to 1000% is earned by different Pharma companies for a same generic medicine in different brands. It is a known fact to all Government Ministry  Officials dealing with it.Our PM goes to foreign countries and invite these pharma companies to come to India and invest here accepting all their terms and conditions. These companies operate openly.Instead of banning these companies or preventing them from marketing or making them bound to keep their profit limited one by fixing either Equal price of all Brands of a Generic or Chemical name of a Medicine as Paracetamol,Government for alluring people and media breakup news ,for cheap popularity started blaming Doctors for this high costly treatment and started regulating Doctors by compelling them  to write Generic Medicines through MCI but whole process is completely false as stated below.

Our Political leader and Babus get hugh donations and gifts in black from these companies,so instead of taking any action on these pharma companies,Brought a new formula that every company will sell many medicines with Generic name or Chemical name as "Paracetamol as Paracetamol only at much cheap rate  and called them "Generic Medicines", for these many tax and duty relief is provided to these companies but Companies are allowed to manufacture Both Generic and Branded medicines separately,as Dr.Reddy laboratory manufactire Omperazole Branded as OMEZ and Generically as Omperazole initially but later on ground of  Doctor or patient or chemist will not understand it or patient will consider it as inferior quality so now these companies are allowed to sell Generic Omperazole with a Branded Name as Omisac,Omitac etc or a new Branded Generic Medicine came in market and replaced Purely Generic Medicine, the golden theme of Government to provide Generic medicine at cheap rate .So No medicine is available on any Chemist counter in India with purely Generic or Commercial name as these are sold as Branded Generic medicines  These So called Branded Generic and Branded Medicines of a company or many companies are sold in retail by same Chemist counter. So Why Doctors are targetted ?How Doctors can write purely Generic medicines as Only Branded Generic Medicines are only available?

Secondly,These Generic Medicines were brought with the idea that their price will be less but infact it is not so for the patient as These Generic Medicines better called as Branded Generic has got very less price in Wholesale but MRP printed on these are almost same as MRP of purely Branded one as "OMEZ" of Dr Reddy Lab is of sam price  as Branded Generic "Omisec " of purely Generic "Omperazole" which is not available at all.So if a Doctor prescribe "Omperazole" then it will be a chemist choice that which Branded Generic Omperazole he  will give as it is available as "Omisac, or "Omesec or Omperaz,he will choose the Brand generic which gives him maximum benefit as these all branded generic are different price of different companies for a single Generic or Chemical Medicine. Although in most branded generic medicines there is a profit of 100 to 1000% from wholesale to Retail price. This was done by politicians and Bureaucrats to benefit chemist and contractors supplying medicines against suspected bribe.Quality of medcine formulation is highly questionable in India as Drug regulators pass many medicines without strict checkup as Inspectors and Babus are bribed so any small company get license easily,so they keep less wholesale price but very high retail price so chemist get hugh profit by providing these branded generic medicine if a Doctor writes only Generic Medicine to a patient.

So our patients or public are cheated in a tune of 100 to 1000% profit in name of GENERIC Medicines too..So if Doctor's writing purely Generic name  of medicine, no benefit to patient will reach until our Government doesnot bound all Pharma Companies to write Generic Medicines with only Generic or Chemical name only not as Branded Generic which is present only in market now and keep their price very low just 10-50% benefit from production to wholesale and the retail market which is now 100 to 1000%.All companies should have same price as well.So Government or Health ministry or its wing should pay attention to this crisis than to blame Doctors only. MCI and IMA which has members mostly as Doctors, should Clear the position before ministers or so called non Medical bureaucrats that doctors are not at fault but it is Government ,its policy is wrong but they as They as Govt blind followers to save their position in these high powered institutes pass gag orders bounding Doctors to write Generic Medcines to please their masters in government knowing fully that it will simply put a good Doctor in Dock and will bring bad name to him/her in public and media for a blunder or crime for which a doctor is at all not responsible.

Frankly speaking word Generic was used for Patented medicine only. The Patented or Generic drugs are introduced in the market by the original company that researched the basic molecule. As Pfizer, which introduced two original molecules - Amlodipine and Sildenafil – and launched them in the international market as Amlogard (Amlodipine) and Viagra (Sildenafil). Being their research molecules, Pfizer had exclusive rights for 10 years based on their patent.  These drugs are called patented drugs and the pharmaceutical company will have exclusive rights to them till the patent expires. After 10 years as the patent period expires, other companies can also market these molecules under their own Brand name or as Generic molecules.  These are called non-patented generic  version of the drugs. There is no difference by law in the quality of generic or patent versions of the drugs but in Price there is hugh difference as "Viagra"(Sildenafil) was introduced @ Rs 600/- during the term of the patent, but the generic version is now available at less than Rs. 25/-.

So, Generic medicines for Hepatitis C and Hiv which cost too much for branded name in foreign countries is available in India at very cheap price but actual benefit doesnot reach to patient or public as Government does not do what it should do.if it doesnot do then it is fine but please leave Doctors for blaming it or even politicians do it then at least our Doctors sitting in MCI or IMA should not be blind to the the above written fact and shall allow their fellow Doctors to earn bad name and repute for their compulsion by ordering them to follow such thing which will further viciate the highly costly treatment as now chemist will give cheap less effective medicines to patient in same high price with no cure or delayed cure of Disease which will further put Doctor in bad name before Public and Media until Purely Generic Medcines at same price by all companies at good quality control is available in market with their production to retail market proft only 10-20% only.
