What is your Identity?

People in this world are generally very intelligent. Intelligence seems like their power and with their intelligence they want to rule everybody around them. If somebody attacks on them then they pledge to destroy that person. They feel they have the power to do anything. A plan is made in that direction. And why not? They have done this in the past and are confident that they will keep on doing like this for all times to come if an attack comes ever again. Are they really powerful? Is their intelligence, their real power?

Behaviour of most people at times is either arrogant or humble depending on how they use their intelligence. When arrogant, then attack happens. When humble, they feel they are obliging people and as if others could do nothing without their help, their advice , their comments, their guidance.

A top industrialist, having the best of political connections feels he can do anything in this world because he has all the money and connections to get anything done. And he has been doing so in the past. Now his only son is suffering from cancer and is on death bed. Doctors are doing their best as per the latest treatments available. He is willing to spend all the money to take him to any part of the world. On his behalf the Government is willing to do anything. But is all of this making him powerful? NO. The fact is that right now, he is feeling very weak and helpless from within. Inspite of all the worldly power he is feeling as if he is nothing. All his power appears useless to him. He starts considering himself worthless. What is his true identity? Is he really something? Or nothing?

Every person in this world looks very busy. What so ever be the reason or the circumstance, they have something or the other to do. When they achieve something, they feel very important and say, " I have done it with all my intelligence." And when something does not happen as per their plan, they are comfortable to blame others around them. They, by and large, are programmed to blame others for their own mistakes. One relevant question here is, "Why do people perform in this world?" The answer is, "Basically to identify their own identity!" Ten achievements in a row gives them a feeling as if that is their identity but in the eleventh one, circumstances are different and people do not accept their authority. So they feel lost. What they have felt as their identity is now not visible and not acceptable. "What is my identity?" is a common question people have in mind at one or the other stage in their life. They want to know about their purpose and role in life. And with this one question, the journey to know self is initiated.

Let us feel the human life from its beginning and its end. When a human being is born, he can not walk, he can not talk, he can not eat on his own, he has no control over his bowel movements and so on. Anybody may come and crush the infant. A stage of total helplessness! That means HE IS NOTHING as an individual at that stage of his life, which is just the beginning. Same is the situation in the real old age, when he is at a stage close to the end of life. Again as an individual, he can not walk, he can not talk, he can not eat on his own, he has no control over his bowel movements and so on. Again, HE IS NOTHING. The fact is that even during life, every person is nothing in front of somebody. Today, one is nothing, and tomorrow after putting lots of efforts, again one is nothing. Even a very influential person feels helpless and NOTHING at times in life, when best of his resources and money can not help him solve his emergency situation. So it is wisdom to accept the basic reality that "I AM NOTHING"?

How To Live, "I am Nothing?"

Once accepted, then we can look into 'how to live I am nothing'. It is very simple, easy & practical.

Once the mind has accepted the basic reality that "I am Nothing" then the need is to understand - how to live "I am NOTHING". I met a person and asked what his name was. He said, "I am John Smith". I said, "Are you sure?" He said, "Of course yes. I am John Smith." "So what does your children call you as?" "Papa", the name has changed. Sister-in-law calls him as brother-in-law; nieces & nephews call him as Uncle, his friends call him as Johny, his parents call him as son, his sister calls him as brother, a shopkeeper calls him as Sir. His boss introduces him by saying, "Meet my Subordinate". His subordinate introduces him saying, "He is my boss" while others in the office say, he is my Colleague. When he goes abroad, he is known as an Americam, British or Indian as they case may be. His next door person says, "He is my Neighbour" and so on. For all scientists on guinea-pig experiments, he is only a Human Being. Strange indeed! How many persons literally call him by his name, Mr John Smith. Hardly anyone indeed. So, is he John Smith or ...........? is a question for introspection.

Observe the life of an actor. Sylvestor Stallion has been given a different name in every movie as per the character he plays in that movie. It is not only the name but the entire personality which is different as per the character. In his real life, he is entirely different perhaps. But while playing a role, he is NOTHING in front of the Director. The Director tells him to perform as a smuggler in a specific role of a movie; he dresses up like a smuggler, behaves like one, justifies the role and comes back to his seat. In the next movie, he is playing the role of a police inspector and then as a poet and so on. He is justifying his specific assigned role and coming back to his base where he is NOTHING. And with a focus to justify his performance, he is not allowing the overlapping of roles. Otherwise, he will not be popular among the masses.

Same way, you have been assigned various roles to play and justify in your life: your role as a son, father, brother, husband, friend, boss, subordinate, colleague, buyer, seller, employer, stranger, patient, tourist and so on. Your performance for all these roles is different. Different as a son & as a father. Different being the CEO of your organization where you must behave as per the organizational needs. And different when you go to a religious place where you even clean the floor yourself. When you go to a school for the admission of your son, you behave totally different. A primary school teacher asks you to wait outside and you are waiting, while in your office people are waiting for you where you feel on top of the world.

But once these roles start overlapping, life becomes complicated and miserable. If you ask your wife to tie a rakhi at your wrist, her brothers won't spare you. You must justify each role as it comes to you and then come back to your base that is NOTHING. Let us learn from car driving. I have asked one simple question during my workshops,"While driving a car how would you change from 2nd to 3rd gear?" Many people explain in different ways but the expert always says, "Come to neutral first and then go to the respective gear." Coming to neutral is extremely important. If by chance you do not plan to come to neutral first then perhaps you are in the process of damaging the gear box. Same way, in life also we must live one role, then come to neutral (I am nothing) and then go to next role and so on. This way, you live beyond 'the disturbing and the threatening EGO which is always making you something or the else against the basic reality where you are nothing.

Prologue:
Dear sisters
Right at the outset I would like to pay my humble tributes to you and your profession and express my gratitude on behalf of all those whom you serve.
Yours is such a profession where there is almost no end to the sacrifice that you make. Most of you work hard; on domestic as well as occupational front! I really admire your patience and salute you for the same. Really you are the chosen ones of the Almighty and hence most privileged.

Secondly I would like to congratulate you for your success in achieving expertise, which is so valuable in healing the ailing mankind.

Now before I go ahead I would like to concede the possibility that you could be knowing much more than the contents of this book. Also, you may be practicing Total Stress Management better than me and you may already be healthier than me in every possible way. But this is my humble effort to share with you what I understand and try to practice!

In view of your supreme importance in patient care and in view of the extremely stressful situations you have to go through, it is imperative that somebody cares for you and your health! So kindly appreciate my concern about your well being and instead of believing blindly or rejecting blindly, consider these contents with open mind, verify and accept or reject on the basis of your own experience.

Total Stress Management For nurses working with critically ill patients.

It is good to appreciate that Total Stress Management (Holistic Health Promotion) requires two aspects to be handled. First; is you and second is your environment.
Let us consider the first aspect i.e. you.
There are five aspects of your being!
They are, spiritual, intellectual, emotional, instinctual and physical! It is needless to clarify that these are not watertight compartments and the classification is for the purpose of simplicity in understanding.
So, the Total Stress Management (Holistic Health Promotion) involves nurturing these five aspects of your personality!

Spiritual health!
When you work with patients requiring critical care, you are facing agonies, anxiety, fear, worry and restlessness. You require tremendous spiritual strength in such situations. I am sure you do not lack spiritual strength. But one simple way to nurture or further develop spiritual aspect is imbibing the concept of unconditional acceptance, love and respect for your self. This helps you to
explore the SOURCE OF LIFE i.e. SPIRITUAL POWER WITHIN YOU. Invoking this power within you is also called as exploring the healer within you.
Therefore learn to ACCEPT, LOVE AND RESPECT, yourselves, unconditionally. This is possible by two means. One is through conscious efforts and the other is through the traditional practice of NAMASMARAN i.e. remembering the name of God or your Guru.
Namasmaran is a simple way of growing from within. However it is ironical that since it is simple and inexpensive people [including me] find it most difficult to practice it! It appears too simple and hence incapable of nurturing us spiritually! But I feel that spiritual growth can really be possible through Namasmaran and it can increase the healing power within you.

How to assess your spiritual health?

There are few criteria by which you can assess your spiritual health and develop it further.

One criterion is the perspective of global welfare. You begin to think and automatically wish the best for one and all! This in itself strengthens you.

Another criterion is fearlessness. It is true that through practice and experience most of you grow fearless even in otherwise dreadful situations. But fearlessness resulting from spiritual growth is nondecremental i.e. without any decline and also, it is devoid of callousness!

Another criterion is your talk.
One of the hallmarks of spiritual growth is revitalizing words. Your words become naturally empowering. You do not have to cultivate or artificially make them so!

One more criterion is your look. Your facial expression and body language naturally reflect affection and care. Your mere presence and your touch begin to heal your patient. In other words empowering happiness begins to emanate through every aspect of your existence.

It must be appreciated that depression and melancholy are infectious. Pain and misery can also be infectious. They can erode you without you being aware of it. Hence you require this power within you to help yourself and everyone around [patients, their relatives, other patients, your colleagues and even the doctors] to overcome stress!

Spiritual growth is additionally important because it gradually reduces two most important causes of stress viz. pettiness and superficiality. Pettiness and superficiality create unending dilemmas, which can really harm you. Hence yet another criterion of spiritual health is broad mindedness and profoundness evident in not getting disturbed by trivial matters!

For the spiritual growth or spiritual health or for exploring the healer within you, AUTOSUGGESTIONS can also help. But thoughtless autosuggestions should be avoided; as you may get trapped in your own petty or even mean ideas.

Following autosuggestions are based on profound philosophical insight. They are of great value.
A] I shall not get angry or feel sad today because it is about past which has gone by.
B] I shall not worry or get restless because it about future which may not come.
C] I shall thank the nature or God for the bounties, because they are many times concealed and not apparent.
D] I shall respect everyone, because global unity is an objective reality and God is present in every one.
E] I shall do my duty honestly because honesty means exploring the universally beneficial element in my work and that would prevent dragging of mind from getting distracted towards fallacious and illusive glamour present elsewhere.

You must be familiar with different prayers, hymns, bhajan, kirtan, pooja, dancing meditation, laughing meditation and many other forms of worship. You may or may not practice one or more of them as per your choice, liking, inclination or faith.

Now let us understand the role of intelligence in spiritual health by an example.

Imagine a swimming pool, with a facility of a nice diving board. Imagine beautiful steps to reach that diving board. Imagine that you can not swim unless you dive!
Now, swimming in the pool after diving wonderfully and enjoying it is like enjoying spiritual health. The steps to reach that diving board are like rationality or intelligence.
Those of us who do not climb the steps obviously cannot swim. Irrationality and blind belief are hurdles in acquiring spiritual health.
Similarly those who climb the steps and reach the diving board, but do not jump from it can also not swim. They get imprisoned in their own intellectual framework. They do not realize its limitations. They are blind to millions of possibilities, which are beyond the range their own intelligence. Such intellectual imprisonment also is a hurdle in acquiring spiritual health.
Climbing the steps, reaching the diving board and jumping from it [getting freed from the enslaving shackles of one’s own paradigms and intellectual frames] help one to be spiritually healthier.
Intellectual health

Now it must be clear to you how all the other aspects of your personality would begin to get nurtured by sound spiritual growth. Spiritual empowerment is like sap of life. Through your crystal clear inner core spring; the globally beneficial thoughts and plans!
For a moment I may sound too theoretical, hypothetical or even impractical or dreamy.
But as you actually blossom more and more from within, you would realize the limitless dimensions of your intellectual and creative potentials. You increasingly appreciate with clarity the role you can play in your own way in global welfare. This consciousness is the fountainhead of intellectual health!

It creates the intellectual zest and dynamism intrinsic to intellectual health. Then you stop undermining yourself in spite of knowing fully well that the world is so vast.

Now you would start getting involved and interested in your job to a much greater extent. You get much more motivated to study the intricacies of your job. You expertise and efficiency starts multiplying. In the midst of humility; you get bolder and inquisitive. Even though asking questions is a royal way to intellectual health the reading books and literature pertinent to Total Stress Management (available for free download on www.superlivng.net) is extremely important and absolutely essential!
Your intellectual empowerment engenders delegation of increasingly important tasks to you by your superiors. This process automatically makes you confident, and smart.
Because of your knowledge; every one begins to need you, love you, admire you honestly and also respect you. Since you are knowledgeable, you no more resent questions of your juniors and your patients. You no more resent questions from patients’ relatives. This is how the whole working atmosphere gets filled with buoyancy, enthusiasm and above all; cordiality.

However intellectual health can be further buttressed and blossomed by specifically conquering the high tech life style, “one up man” ship, knowledge of diseases such as cancer, diabetes, hypertension, ischemic heart diseases etc., information mania etc, which all increase stress.
1. Adopt high tech life style with due sense of proportion and with respect to the comfort it would give you.
2. One up man ship or competition should be adopted for achieving excellence. If there are petty pursuits, then they can ruin your happiness easily.
3. Knowledge of diseases can produce deep impact on mind and lead to neuroses and phobias. These phobias can spoil the life completely. Therefore it is essential to “ride” over the knowledge! Here spiritual health can really help you as negative thoughts are effectively overcome by spiritual bliss, (though it is not easy!). Secondly you have to learn to appreciate that every thing we read or know, is not complete in itself. Even if the information about the diseases is correct the human spirit is invincible and is “more correct”! Hence do not get bogged down and stressed by threatening information about the diseases published, broadcasted or telecasted; whether in scientific journals, text books or in media.

Dealing with information explosion properly, makes you humble and realistic also!

This is well illustrated in the anecdote of Mahatma Gandhi’s three monkeys.
After one reads or hears about these three monkeys, one tends to ask, “Why should I not hear, see or talk bad if I really am convinced about it? Does not it amount to being a hypocrite?”
The answer is simple but very useful in acquiring intellectual health!
What we hear and interpret is a function of limited range of hearing and seeing and limitations of the processing of that information based on past experiences, in our brain. Hence to feel that it is the truth and declare it amounts to being unrealistic and arrogant. It is fallacious. Therefore we should and if at all; humbly state as “our impressions” and not claim it to be absolute truth.
Understanding this scheme gives you the knowledge of perceptive [intellectual] error that leads to stress and its correction.
Intellectual health manifests in globally beneficial perspective, policies, plans and actions in your field, which would blossom not ny the patients, but everyone involved in healthcare; directly or indirectly!

Emotional health

Every one knows the different positive and negative emotions. It is very easy to say that we should overcome fear, anger, hatred, depression, jealousy etc. It is also very easy to advocate positive emotions such as peace, happiness, love and so on.

But the important difficulty is how to actually be able to do it?

Human brain itself is such that those parts involved in emotions are hardly under the control of the parts instrumental in thoughts! Moreover there is no clarity about parts of brain [if any] related to spiritual growth. Hence even as spiritual health and intellectual health can substantially ward off many emotional disturbances, most emotions do not always follow the suit of spiritual and intellectual processes. They have good deal of autonomy and volatility. Hence additional specific measures have to be taken to nurture emotional health.

We have to understand here that emotional health depends largely on soundness of our limbic system and autonomic nervous system, though it is influenced to a certain extent by central nervous and hormonal activities.

Now what is the practical relevance of these facts?
The relevance is that we can practice following pranayamas to tone up these parts and acquire high degree of emotional health!

KAPALBHATI
KAPAL means forehead and BHATI means light or brightness.
You are supposed to do active and moderately forceful abdominal expiration and you are supposed let the inspiration take place passively. Do it rhythmically but not jerkily. Count the strokes of expiration. Practice about twenty strokes initially and gradually increase the number up to five hundreds to one thousand in comfortable manner.
Do not practice after eating food.
Do not practice if you have wound [e.g. surgery], hernia, hydrocele, or any inflammatory condition of your abdomen.
Do not practice in pregnancy.

Initially you may find it little difficult, but a few days of practice would make you comfortable.
I would not like to make any tall claims. But you may find out for yourself whether you lose weight, feel energetic, feel buoyant, feel younger or no. In addition find out whether you have stopped palpitating in minor adversities or imaginary fears. Verify whether unwanted perspiration stops or not. Find out whether you get any benefits on your lipid profile or blood sugar. Emotional health has clear cut positive impact on your instinctual and physical health. This can be called the byproduct of the emotional health!

BHASTRIKA
The meaning of the BHASTRIKA is bellows.
You are supposed to breathe in and out forcefully mimicking the action of bellows. The rhythm of the bellows is necessary but it is important to avoid jerk.
Do not practice at least five after food.
Do not practice in case of hernia, hydrocele, wound or inflammation of any nature in thorax or abdomen.
Do not practice in pregnancy.
KAPALBHATI AND BHASTRIKA if practiced cautiously may help in early stages of certain malignancies.

UJJAYI
UJJAYI means victory.
Breathe out slowly, forcefully and fully. Now partially contract your larynx i.e. the region of voice box and breathe in with a high pitch
in-blowing sound.
There are no precautions and restrictions for UJJAYI except presence of lesion in respiratory pathway itself.

BHRAMARI
BHRAMARI means related to BRAMAR, which means the familiar insect sucking honey from flowers making typical humming sound while flying. Since you produce similar sound in this pranayama it is called BHRMARI.

Hold your index fingers on forehead, middle and ring fingers on the eyes and little fingers below the eyes and thumbs pressing on ears and breathe in maximally without jerk, and then breathe out slowly through voice box and nasopharynx producing humming sound through out your expiration. The vibrations produced from within have healing and revitalizing effect on everything within the head!

ANULOM VILOM
Breathe in through one nostril and breathe out through the opposite nostril. Now breathe in by the opposite nostril and breathe out through the original nostril.
This is one cycle. You can repeat this ten to twenty times as per your convenience.
Do not give jerks. Do not practice after food for at least three hours.

Later three pranayamas can be practiced in pregnancy or any other condition of the abdomen or thorax. You can practice them in sitting or any other position.
In this pranayama, since holding of breath is not advocated you can practice it even when you are traveling.
Please practice and verify whether these techniques make emotional health sound with several additional benefits on you physical health.

We have to pay attention to our instinctual health and physical health for overcoming unpleasant sensations and consequent emotional disturbances (e.g. nagging, irritating, nauseating, disgusting sensations arising from within our stomach, duodenum, ileum, ascending, sigmoid and descending colon, rectum, anal canal, urinary bladder, uterus, ovaries, muscles, fascia, tendons, ligaments, joints, blood vessels, pregnancy, delivery, lactation, menstruation, menopause etc.)

Few simple tips to which, you can add many more according to your experience can contribute to your emotional health even though they are most not essential.

You must sleep well or else you get more irritable
Take adequate rest as other fatigue makes you depressed, withdrawn and negative.
Observe cleanliness as unclean habits due a variety of reasons can cause allergies, infections, psoriasis, lichen planus and other nagging skin disorders. They can really disturb you peace and make you miserable.
Use well fitting, comfortable and suitable clothes as bad clothing can create difficulty your movements. It can spoil your body image and self-esteem. Ill-fitting clothes distract your attention and reduce your efficiency. Make use of natural perfumes and pleasant colors to brighten up the atmosphere and make it cheerful. Ornaments and cosmetics too may have a “cosmetic” role in acquiring sound emotional health!

Try to practice following few points and see if you start feeling better. My experience tells so.
1] You can charge the atmosphere by merely greeting the people properly and giving them suitable gifts. For that you have to accept them unconditionally, love and respect them. Pay attention to them and their ailments with total focus. Such bond actually revitalizes you. When you see the feelings of gratitude and solace, your emotional health begins to improve.
2] Your patients need your love. They do not appreciate condemnation, criticism and value judgment.
3] Appreciate people. Always complement by words such THANKKS and GOOD etc. You get appreciation if you give!
4] Apologize profusely and naturally for even so called small mistakes and errors.
5] Always say you agree during your dialogue even if you do not. In the course of well begun dialogue you can convince your point.
6] Spare time for certain hobbies, such as music, knitting, painting etc. so that there is no monotony and you can pleasure of the variety!
7] Find out the degree of pleasure from its intensity, its duration and its impact on the society. This certainly helps you to decide what is more important than the other. This is called PRIORITIZATION.
Once a thought or a feeling or an action is judged as the most important objective it becomes easy to ASSERT yourself.
After you ASSERT just recollect the moments of ecstasy of maximum intensity, duration and that simultaneously benefited others.
Relive ASSERTION and CONSOLIDATE your priorities. This is POSITIVE REINFORCEMENT.

Now recall the thoughts, feelings and actions that gave you maximum sadness lasting for maximum duration and hurting or inflicting others. This would help you to avoid the same on priority basis and is called NEGATIVE REINFORCEMENT.
When you regularly practice positive and negative reinforcement the WELLNESS CYCLE begins. Wellness cycle means increased experience of active peace, intellectual sharpness, happiness, healthy instincts and satisfactory physical activities. WELLNESS CYCLE destroys the deleterious effects of vicious cycle or distress cycle.




Instinctual health

Spiritual, intellectual and emotional health forms the basis of instinctual health. But you can be benefited more from the following adjuvant measures as well!

Biologically speaking, we the humans also inherit the instincts, which are in mammals and birds.
These instincts are integral to life and largely related to neurohormone and hormone secretion and metabolic activities. Hence if instincts are not satisfied the stress produced is tremendous and far more dangerous than mere emotional stress and hence you have to overcome it “instinctual stress” to acquire instinctual health!

1] Survival
Survival is the most basic instinct. With technology survival has become easier. But with cut throat competition, increase in crimes, frauds, irregularities and corruption, simple person are getting increasingly insecure. The advertisements of life insurance, health products, medicines etc exaggerate the dangers and increase the insecurity even further. The result is increase in suicides and murders. It is important to be spiritually, intellectually and emotionally strong by learning, teaching and publicizing the Total Stress Management for conquering the fear of survival and thereby acquire instinctual health.

2] Hunger and thirst
They are important for survival. We call them instincts because they are extremely strong drives determining the behavior. If they are not satisfied they can unleash violence of unimaginable magnitude. Secondly eating and drinking behavior has tremendous influence on your total health!

a] Do not fall prey to the aggressive advertising campaigns. Use your own discretion and common sense.
b] As far as possible eat home made food,
c] Avoid excessive consumption of any one type of food such as ice creams, chocolates, deep fried foods etc. Do not drink excessive soft drinks, beer or hard liquor.
d] Include wheat, patni, nachani, bajara, jawar, corn and also neem, karela, bel, Amala, ginger, pudina, cocum, tulsi, jamun, cow milk, butter milk, green leafy vegetables, salads, seasonal fruits, methi sprouts, alfalfa [lucern] sprouts and different oils in your diet. Let there be variation and moderation.

Sexual instinct
Odd hours of working, excessive time consumed in commuting, lack of sleep, lack of security etc. can adversely affect the sexual drive and capacity. Frigidity, impotence and such many problems can arise and spoil your life completely.

It is not possible to discuss every problem in detail, but I would request you to kindly pay serious attention towards your and your partner’s sexual satisfaction (You may refer my books and articles on sexology available free download on my website www.superliving.net).

Herd instinct
Herd requirement is instinct to live together. The animals habituated or rather biologically bonded with instinct to form herd, go haywire if they are removed from the herd or lost from the herd. Keep yourself in good company. What is good company? Good company is one that reminds and reorients you to your inner power, magnanimity, divinity and vitality, and never drags you to pettiness and superficiality!

Habitat
Habitat in simple words means the over all the atmosphere to which the animals deeply attached. We should try to relive our childhood conditions and keep our roots alive. They nurture us.

Defecation
Polished cereals, white bread, eggs and other food stuffs, which do not contain fiber, tend to reduce the contractility of gastrointestinal tract and may cause constipation. Sedentary habits further aggravate the situation.
So use more fiber in diet by using green leafy vegetables and salads. Take suitable exercise regularly.

Movement
It is proved by experiments on laboratory animals, that the restriction on movements causes tremendous stress. Sedentary individuals suffer from hypokinetic stress. You can overcome such stress and acquire instinctual health by walking, jogging, trekking etc. Practice frequent stroll and change positions frequently.

Physical health
What is physical health? Apparently the answer is very very simple. But Strength, endurance, flexibility, agility, good body proportions, good biochemical blood profile, normal working all parts of body, reserve strength are all important indicators of physical health. From holistic point of view, not merely the condition of your body, but the actions of your body; indicate physical health. So in short what does your brain do as part of your body? What do your eyes see or read? What are ears listening to? What are your hands engaged? So it is necessary to ask such questions about every part of body so that you get a clear idea about your physical health.

Let us note some tips for excellent physical health.
1] Drink lot of water in morning. You may choose to drink warm water if you have tendency for constipation.
2] Splash your eyes with cold water at least twenty times each.
3] Drink a cupful of herbal juice. Herbal juice usually contains neem, harada, karela, pudina, amala, tulsi, adrak and rock salt for taste.

The advantages are many. You have to experience them.
4] Massage your gums, palates, and rest of the parts of your mouth. Use ayurvedic tooth paste or powder for brushing.
5] Bath should ideally be with cold water. It is rejuvenating. But you can use any water that suits you.
There are prayers for reciting in morning, during brushing, during bath and before breakfast. You can combine physical and spiritual development.
6] Exercises: Jogging, walking, swimming, spot jogging, dancing [classical as well as western dance] surya namaskar, yogasana, karate, kung fu, judo, silk weaving exercises, aikido, sit ups and many aerobic and anaerobic exercises , sports, trekking, mountaineering are not available all. But you choose some of the varieties suitable to you.
There many exercises which we have devised for sedentary workers. These are vakrasana, chakrasana, vrukshasana, brahamudra, exercises involving, neck, shoulders, hands, knees, feet and so on.
There seven exercises that can be used to improve eyesight and even get rid of the spects.
a] Start slowly and gradually b] Do not give jerks c] Take care of your tendons and ligaments d] Be cautious about your balance so as prevent fractures e] Do not perspire excessively f] It is safer to be in group
g] Take rest for a day or two in a week.
7] Massage is very useful. It has an inseparable element of emotional soothing. In the evenings the head massage is especially useful. You can do it yourself. You can use Brahmi oil and get the additional advantage.
Massage eyes, face etc.
As far as body massage is concerned, it may not be feasible to take it every day. But at least once in a week whole body massage can do wonders to your health. You can use any oil of your choice and also with consideration to the season. Massage your feet twice or thrice a week.
Regular massaging ensures flexibility of the small joints and adequate circulation and micro-circulation in the feet. This is especially useful in diabetics who are more to develop vasculopathy and resultant damage in the feet, in the form of necrosis and gangrene.
Wash and massage face two or three times a day. Massage your eyebrows and eyelids. Wash your mouth at least thrice a day during duty hours and if feasible brush teeth at least once. Do exercises of tongue such as rolling the tongue up, down, left right etc, inside the mouth.

Emotional disturbances are reflected through body posture. Body posture in turn can affect your emotions. Therefore erect posture is preferred whenever possible. It compliments the TSM. It is important to sit with your back and loin touching the back of the seat. This reduces undue strain on your back and reduces the chances of backache.

Practice SIMHAMUDRA which involves opening the mouth wide open and stretching the tongue out of the mouth and hold it for 10 to 15 seconds.
Practice BRAHMA MUDRA, in which you turn your head to all sides similar to Lord Brahamdev.
Practice NASIKAGRA DRISHTI and BHRUMADHYA DRISHTI in which you focus your eyes on tip of nose and middle of the eyebrows respectively.
Practice jaw relaxation in which you simply allow the jaw to hang. But it is not simple!
Do flexion and extension exercises of your arms, elbows, hands, fingers, ankles, knees, hips, back, abdomen as and when you get time.

To end, the most important thing is; NAMSMARAN and if you can not do anything else at least practice NAMASMARAN.

Improvement in economy through holistic education is pivotal to Total Stress Management i.e. empowerment and blossoming of an individual, school, nation and the world.

If you analyze the poverty and associated globally prevalent issues leading to ubiquitous stress, with a holistic perspective, then it would be clear that the innate core (the spiritual domain that imparts universal perspective and globally beneficial outlook) and the productive domain (that empowers); of people concerned with education; are grossly neglected and remain perennially deficient.

Mainstream Education System and the courses and careers in it; revolve around and serve the grossly petty and superficial considerations, motivations and interests (I am referring to what I have been observing in India for last half a century and guess that it is not much different in other parts of the world)! This state of affairs is strongly protected and strengthened by the political elements with similar interests! Hence laws, rules and regulations develop accordingly!

Steps to rectify

1. Working physically in different ways and earning [productive domain] in itself, is not a burden. It is an opportunity to grow from within. It is an opportunity to develop self esteem. It is an opportunity to become self sufficient.

2. This leads to self sufficiency in schools. They do not have to depend on heavy fees or federal grants or donations. This makes the education institutions
accessible to all and self sufficient!

3. Through productive domain the hypokinetic stress, emotional stress of being dependent and intellectual stress
of excessive memorizing would be averted.

4. Due to productive domain, the dropping out due to lack of earning [as is common in case of millions of students in many parts of world] and then turning into helpless, vulnerable and cheap child labor would come down.

5. Being empowered, the students would not go through the stress of unemployment and turn into helpless, frustrated mental wrecks or criminals.

6. The emphasis on productive (and hence psychomotor and practical aspects) would bring down the necessity to “copy” and associated crimes and corruption in procedures of examinations, certification, providing grants presently rampant in many parts of world.

The caste based reservation for education, jobs and other faculties; responsible for social divide and strife in many parts of the world; (especially India) can be peacefully and advantageously done away with, by consensus!

This is only a brief sketch of the potential advantages of productive domain.

The practical aspects of productive domain can include several crafts and skills and arts and their sale according to the situations. For example, planting of
medicinal herbs, rearing of cows, home flower gardening, production of chalk
sticks, carpentry, pottery, arts such as music, dance, painting and others.

Tips
1. The productive domain should be an essential ingredient of Mainstream Education System, but no particular job should be enforced.

2. The productive domain should constitute only about twenty five percent of the school time.

3. The teachers and others should participate in the productive domain.

Warnings
1. Administrative aspects such as infrastructural development have to be done with professional expertise.
2. It should be done with due care and concern for the physical capacities of the
children and should not be painful and troublesome.
3. All this should be done with a clear aim of all-round development of the students,
Teachers and other concerned.

The True Spirit of India: Dr. Shriniwas Kashalikar
India is not merely a geographic portion of the earth. She is not merely rivers and mountains or castles and palaces! She is neither the riches nor the poverty of people.

India is nothing else but the incessant ongoing search of truth and realization of Truth from time to time throughout the millennia!

Rama, Krishna, Buddha, Mahatma Gandhi, Vivekananda and the likes; are visible examples; and their biographies remind us of; “The omniscient, eternal yet invisible spirit inherent to Search of Truth and Self Realization of many seers; unknown to documented history! This is true spirit of India!”

Let us always remember this eternal, yet invisible spirit of India and be happily thankful to it for ever. It is said that NAMASMARAN can make this possible and make your life really satisfied and fulfilled! You can verify this statement!

Healthy Aging: Dr. Shriniwas Kashalikar
CONCEPT OF HEALTHY AGING:


Before we consider the healthy aging, let us first consider in brief the concept of HOLISTIC HEALTH for healthy living!

HOLISTIC HEALTH
Holistic health in adult human beings is nothing else but ongoing movement towards increasingly accurate perception, appropriate feelings and precise actions, with increasing pleasure, power, prosperity and profundity.
Perception, feelings and actions are also expressed by terms Cognition, Affect and Conation respectively.
A] Cognition
In short, cognition means processing [appraisal] of a huge variety of sensations impinging on us. You can appreciate the importance of this faculty if you consider the two important facts viz. 24 hours of the day and twelve months of the year, every one of us gets sensory inputs in the form of physical, instinctual, emotional and intellectual stimuli, which may be painful or pleasurable in the form of a variety of personal, familial, social, national and international events. Cognition involves sensing, interpreting, memorizing, questioning, pattern finding, analyzing, correlating, synthesizing, contemplating, understanding, appreciating and hypothesizing on the basis of this information input.
2] Affect
Affect means the effect of the variety of stimuli which, may impinge on us. The affect is in a way, “sandwiched” between cognition or perception of stimuli and the conation or response.
In general it involves the activity of various parts of nervous, endocrine and metabolic changes. The affect is usually experienced as a] confidence, enthusiasm, pleasure, happiness, love, attraction, patience, comfort etc. or b] sadness, jealousy, hatred, depression, inhibition, diffidence, loneliness, listlessness, impatience, discomfort etc.
3] Conation or action i.e. response
Conation is the response of an individual. This can speaking, writing, listening, watching, and other interactions usually referred to as behavior.
In traditional parlance this was called fulfillment of four PURUSHARTHAs, viz. DHARMA, ARTHA, KAAMA and MOKSHA through practice of SWADHARMA.
DHARMA refers to accuracy in perception, affect and conation. This leads to ARTHA i. e. acquisition of material gains, followed by KAAMA i. e. satisfaction of the mundane desires [DHARMA AVIRUDDHAH KAAMAH i. e. the desires which are not against the DHARMA i.e. principles of global welfare.]. KAAMA is followed by MOKASHA i. e. liberation from all sorts of bonds.

In short it must be appreciated that for most of us holistic health refers to ever increasing and appropriate satisfaction of physical, instinctual, emotional, intellectual needs and fulfillment and happiness arising from it.

Healthy Aging:

By the word ageing we generally think of an unpleasant process associated with dysfunction and disability. We think of a phenomenon full of dependence and helplessness. It seems to indicate only cumulative wear and tear due to micro insults and time dependent programmed loss of structure and function. It seems to imply physical, psychological, intellectual and social incapacitation. It seems to involve choking of charm, routing of romance, decay of dreams, annihilation of aspirations and end of new beginnings. It seems to be an entity that signals us to wind up every thing get ready to quit this world- the world that we so much long to live in.

So let us study what is underlying this process of ageing. But before we do that we have to distinguish it from the handicap or dysfunction that results from diseases and the changes resulting from diseases and the natural process of ageing, i.e. one which would take place in the absence of diseases.

One can appreciate that such distinction is quite difficult. Most of us suffer from ailments such as infections, malignancies, diabetes, IHD, as well as environmental onslaughts in the form of ultraviolet radiation, heat, cold, pollution, food additives, allergic substances, physical and chemical injuries etc apart from psychological inflictions!

But still the ageing that results in absence history of gross or major diseases roughly represents the true ageing and is generally attributed to two basic facts

1] Genetic program:
Decides the range of the longevity of life for a specific species and determines genetic program by virtue of which there is time dependent loss in structure and function. Genes program senescence and death.

2] Accumulation of injuries or micro insults:

Since very little is understood in this respect there are many theories based on these two tenets to explain the ageing. Since they focus on a specific change, they are inadequate. But together they do give us some insight into the process of ageing.

2] THEORIES OF AGEING:

A] The Error Catastrophe Theory:
Random increase in errors of protein synthesis i.e. defects in transcription and translation. But these seem to be post translational modifications.
Further, in senescent cells accumulation of misspelled proteins is not found and Induction of synthesis of erroneous proteins experimentally does not give rise to ageing.

B] Somatic mutation theory:
This suggests that there is age associated increase in chromosomal aberration.
This is suggested to give rise progressive increase in inefficient cells leading to organ dysfunction.

C] Free radical theory:
Accumulation of metabolic waste products and radical mediated cell damage e.g. hydroxy peroxide, aldehydes and ketones, superoxide radical, singlet oxygen and hydrogen peroxide..
Lipid peroxidation at cellular level is not demonstrated in ageing.
And antioxidants are not as yet proved to be useful in delaying ageing.

D] Ionizing radiations and such other environmental factors may cause cell injury.

E] Cellular dysfunction in tissues such as CNS, endocrine and immune system and influence other systems. Thus decrease in synthesis, increased breakdown, decrease in receptor number, decrease in H-R combination, decrease in HR internalization etc.

F] Sedentary jobs, late night parties, noise, crowding, stress of highly competitive life, uncertainty of socio-political surroundings, consistent and protracted denial of the recognition and appreciation in life, habits such as excessive smoking, lack of enjoyable job, lack of promotions and incentives in jobs etc. and the plethora of stressors either not thought of or not given due importance and hence not mentioned in the text books; immensely influence the aging process; and are described; in my books viz. “Stress: Understanding and Management” and Conceptual Stress: Understanding and Management”.


3] VARIOUS CHANGES TAKING PLACE DURING AGEING PROCESS

Overall changes in the body are decrease in the height, increase in the length of nose, increase in the length of ears, decrease in the circumference of neck, decrease in the circumference of thighs, osteoporosis, bending of the vertebral column causing kyphotic deformity, reduction in the periorbital fat causing sinking of the eyes etc.

a] PHYSIOLOGICAL CHANGES
Broadly speaking, maximum capacity, tolerance, peak performance and reserve are reduced.
I] Matrix:
Collagen becomes stable, more rigid, more insoluble and also reduction in its content and increase in its degradation. In addition there are changes in proteoglycans and plasma proteins. This is due to damage due cross linking, less formation of collagen, reduction in the elastin. Fascia, tendons, ligaments, bones, joints, and peripheral vascular disease become rigid. Skin becomes dehydrated, devoid of subcutaneous fact and less elastic thus skin becomes wrinkled, dry, pale due to reduction in capillary bed, more susceptible to injury.
II] Blood:
Red bone marrow is replaced by yellow bone marrow in the long bones first, flat bones and then vertebrae. Physiological reserve capacity for erythropoiesis and leucopoiesis is reduced.
III] Gastrointestinal tract:
Teeth : Enamel, dentin and cement all show decline. Loss of teeth due to caries, periodontal infection and reduced masticatory efficiency.
Weakness of cricopharyngeus, reduction in presure gradient and inability to relax the lower esophageal sphincter cause dysphagia.
Stomach: Age related atrophy of mucosa causing achlorhydria. Pancreatic lipase reduced causing streatorrhoea.
Motility of GIT is reduced: Hence there is tendency towards constipation. In Ayurveda this is categorized under VAATA VRIDDHI.
Intestinal lactase activity is reduced and villi are reduced and absorption is reduced.
Liver cells reduce in number fibrous tissue increases and size of liver cells increases.
Protein synthesis and microsomal mixed oxidase activity required for metabolism of drugs and steroids become less. Thus functions are reduced but since the reserve is great the tests are in normal limit.
IV] The immune system:
shows decrease in the T cell activity, decrease in AB production and presence of auto anti bodies due to reduced tolerance to antigens formerly recognized as self. There is susceptibility to infections. Since immunological -surveillance is believed to eliminate neoplastic cells there is higher incidence of cancer. Following stress of bereavement there is steep decline in cell mediated immunity and this causes death following death of spouse or any other near / dear one.
V] Reproductive system:
The hormonal secretion reduces and there is stoppage of ova formation, ovulation in females and there is gradual reduction in the spermatogenesis causing reduction in the sperm count.
Females show a distinct and identifiable change in the form of menopause where has such change is not distinct in males.
VI] Central nervous system and special senses:
Atrophy of the brain and neuronal loss. Accumulation of lipofuscin and loss of synapses and dendrites. Cholinergic deficit is demonstrated in Alzheimer's disease and Dopamine defect is demonstrated in Parkinson's disease. Milder form of cholinergic deficit may be responsible for commoner forms of senile dementia and milder form dopaminergic deficit may be responsible for milder form of hypo-kinesia seen in old age.
There is deficit in autonomic responses leading to postural hypo-tension and impairment of temperature regulation
Decrease in sleep: There is difficulty in getting sleep as well as there is tendency to wake up during night and waking up early.
Presbyopia, cataract, rise in intraocular pressure. Presbyacusis i.e. difficulty in hearing causes difficulty in understanding speech and localization of sound.
Sense of smell and taste also decline with age but are not studied adequately perhaps because they do not cause serious functional deficits.
VII] Endocrine system: There is decrease in sympathoadrenal axis and there is reduced tolerance to stress.
VIII] Cardiovascular system:
In those where there is no atherosclerosis, there still reduction in elasticity of aorta. Increase in systolic and pulse pressure. But no change in diastolic pressure.
Atherosclerosis, Atrophy of myocardium, Accumulation of lipofuscin, Fibrosis, Deposits of amyloid, Contractility of heart is diminished, contraction and relaxation time are increased, Decrease in ventricular compliance, stenosis of aortic and incompetence of mitral valves, Number of pacemaker cells is reduced and so, responses to sympathetic as well as parasympathetic stimulation reduce and this causes postural hypotension.
Maximal HR during exercise is reduced in aged but cardiac output is maintained by increasing stroke output.
IX] Respiratory System:
Microscopic changes:
The alveoli become flatter and narrower and ducts enlarge. Walls of the alveoli become thin, capillaries decrease in number. Alveolar surface area decreases by 4 % every decade after the age of 30. Pulmonary blood vessels show age related increase in wall thickness.
Functionally there is decrease in total and timed vital capacity. Increase in residual volume. Due to reduced elastin lung compliance increases but compliance of the total respiratory system decreases due rigidity of chest wall after the age of 60 years.
Due to loss of elastic recoil the pressure which has to be built during expiration from the alveoli so that air from alveoli forces open the airways, is decreased. Due to this there is tendency of the airways to collapse. This tendency to collapse increases during expiration when expiration must become active.
Due to decrease in capillaries there is reduction in diffusion.
The response to hypoxia and hypocapnia are reduced.
Thus ventilation, diffusion and regulation are all impaired in elderly individuals.
X] Muscles :
Atrophy, reduction in contractility, decrease in tone, cataract, hernia and rupture of inter-vertebral disc etc.
XI] Excretory system :
Kidneys reduce in size, renal plasma flow and GFR reduces 10 % per decade after 30, nephrons and their secretory and absorptive functions reduce and fibrous tissue increases. Kidney vessels show age related changes irrespective of hypertension.
It has been hypothesised that due to high protein diet there may high solute load on the renal capillaries leading to chronic dialation of the capillaries leading to extravasation macromolecules in renal glomeruli leading to mesangial reaction causing renal damage.

b] PSYCHOLOGICAL CHANGES:
Frustration, depression, fear, anxiety, insecurity, loneliness, vacuum, dejection, self pity, sadness are some of the hallmarks of old age.
With ageing there is increasing restriction on the kind of enjoyments one is accustomed to and this can lead to frustration or depression.
The increasing chances of death especially due death of the contemporaries cause alarm, concern, anxiety and fear of death, diseases and debility.
Due to lack of job after retirement or physical inability there is always a sense of insecurity.
Lack of the children's company due to their being increasingly independent gives a feeling of loneliness. This can happen due to children going abroad or away from home for their jobs.
Since most of the people of younger age groups have their own pre-occupations the elderly are left out. More over the ideas, choice, preferences and other areas of interests differ and hence the elderly are cut off from younger generation. This distance gradually increases with advancing age.
This leads to vacuum, dejection, self pity and sadness.
Thus overall the old people tend to become increasingly melancholic.

d] ECONOMIC CHANGES
The ageing almost always associated with reduced income and increasing economic dependence on the others.

c] SPIRITUAL CHANGES:
In India we find that there is an increasing tendency to visit temples and SATSANG. This is because they want a certain kind of solace which they do not get from day to day life and routine activities. This is also to fill the vacuum created due to retirement, and to get company of those who sail in the same boat.
This has given to a widely prevalent yet totally wrong notion that the spiritual pursuit should be undertaken at old age.

But the courage, alertness, enthusiasm and other physical and mental faculties which are extremely important to undertake spiritual pursuit [i.e. growth of consciousness] considerably diminish in old age.

Old people become some what disinterested and indifferent towards life. But this should not be taken as detached attitude described in Geeta. Old people are generally detached because they accept that they cannot do anything to either prevent things which they do not like, or to make things happen which they like. Thus their detachment is out of giving up, or acceptance of defeat.

Some elderly individuals develop ascetic thoughts due to losing interest in the routine life because they are tired of responsibilities and dynamism required for their job or profession. This may mean reduction in income. But this has nothing do with financial sacrifice. It is only fallout of exhaustion.
It is not a spiritual virtue of selflessness.

Some individuals lose interest in the surroundings due to decreasing sensory perception and decreased mobility. They become shy about their deficiency. So they avoid socialization. Gradually they become increasingly self centered and selfish. They become over-concerned about themselves and less sensitive to others' problems. They become more rigid, adamant and at times obstinate. In an attempt to assert themselves; they become unreasonable and erratic.

But the impression, which we get from the foregoing, is quite inadequate and therefore wrong. Further it is dangerously pessimistic and depressing. The horror of ageing, as made out to be by merely observing certain physical or psychological changes without realizing that there is a vast difference between the ageing of the animals and that of the human beings, only creates fear about ageing. As a result; instead of helping the aged; we expedite the process of ageing and its ill effects; not only in the elderly; but also in the younger minds by creating fear psychosis. I have coined new terms for it viz. gerontophobia, and genrontoneurosis.

The depressing notion about ageing; fails to notice the fact that while ageing, a person accumulates not merely micro insults and injuries but simultaneously accumulates a variety of experiences, associated with increasingly appropriate and accurate interpretation of those experiences and increasingly accurate responses to challenges in life.

Ageing is also associated with greater and greater degree of fulfillment, satisfaction, and contentment due to achievements in life, success in the aims / ambitions / aspirations. In the lesser fortunate ones it is associated with happiness of having procreated and seen children and grand children grow. It is associated with experience of their love. Even in the least fortunate ones it incorporates the satisfaction that one has lived [has not died young!].

Further it has be realized that while ageing the aged individual gradually learns the inevitable nature of ageing and death and hence gradually learns to accept them uncomplainingly and gracefully.

Age is respected in Indian culture. Thus age brings extra care and respect to the individual. He or she gets more services and comforts in preference to the younger lot.

Let us clearly and definitely understand and appreciate without any ambiguity that the meaning of the word ageing is not merely becoming old but also developing and maturing. Aging is actually your passage or sojourn; through the body; full of wonderful learning and blossoming! It is a passage of merging with your true self! But it is essential to catch and hold on to this thread of wisdom of pursuing your true self right from childhood through the practice NAMASMARAN with topmost priority, beside other physical measures described in fitness and yoga guides. This can help us overcome our fear of old age and help the aged people to help themselves also and others and improve the quality of everyone's life aged as well as; to be aged.

BUT WHAT IS NAMASMARAN?

Namasmaran means; remembering the name of God, Guru, great souls; such as prophets and holy objects such as planets and stars. It may be remembered silently, loudly, along with music, dance, along with breathing, in group or alone, either with counting by rosary (called SMARANI or JAPAMALA) or without counting. The traditions vary from region to region and from religion to religion.

However the universal principle underlying NAMASMARAN is to reorient your
physiological being with your true self. In fact while reorienting with true self you aim
to establish and strengthen the bond or connection; between; your physiological
being; with your true self. You aim at reunification with yourself!

Since remembering your true self is the pinnacle of or culmination of individual
consciousness, and individual consciousness is the culmination of every activity in life,
remembering any name of God or Guru (any symbol of your true self) is equivalent to opening the final common pathway for the individual consciousness associated with
every possible activity to get funneled into or unified with objective or cosmic
consciousness.

Thus NAMASMARAN is in fact the YOGA of YOGA in the sense that it is the
culmination of consciousness associated with every possible procedure and technique in
the yoga that you are familiar with. It is the YOGA of YOGA because it is the
culmination of consciousness associated with all the activities in the universe, which it
encompasses as well! It is YOGA of YOGA because everybody in the world irrespective
of his/her tradition and the beliefs; would eventually, ultimately and naturally reach it
it; in the process of liberation. Even so called non believers also would not “miss” the “benefit of remembering the true self through one symbol or another”!

Just as NAMASMARAN is YOGA of YOGA it is meditation of meditation also! It is the natural and ultimate climax of every form of meditation.

These facts however have to be realized with persistent practice of NAMSMARAN and
not blindly believed or blindly disbelieved with casual approach!

In short NAMSMARAN is super-bounty of cosmic consciousness for every individual to
realize it (cosmic consciousness)! This is truly a super-bounty because a person who
experiences it, rises above mercenary, commercial and even professional and
charity planes and manifest super- transactions in his or her life!

These are just few observations to give rough idea about what is NAMASMARAN.
NAMASMARAN is an ocean of bliss. Its true meaning is beyond description in words and has to b experienced, most preferably, by billions!

1. Accept, love and respect your
patients unconditionally: it
promotes healing
Consider this experience of mine in a clinic. I had
gone to a pathologist with another friend of mine to
collect the blood reports. We were waiting in the
reception. My friend’s face was full of anxiety and his
eyes reflected a strange feeling of helplessness and
suffocation. Being close to him I was aware of the
financial and other stresses he was going through. I
was also aware as to why he was undergoing different
blood tests.

After some time full of tension and pressure the
pathologist called us.

As soon as we entered he turned ferocious and started
scolding my friend, for indulging.

He was so precipitous and incessant that my friend could nothing but only listen in shame.
After the pathologist finished his bombardment his eyes began to emanate the contempt and pity.
The pathologist had satisfaction on his face for having fulfilled what he considered his duty.

My friend was drowned in self-pity and guilt. He appeared far sicker now than when he had entered the lab.

It was becoming unbearable now.

Some how I changed the topic, finished the formalities of payment, thanked the pathologist and came out.

My friend said, “I would prefer to die in dignity than getting harassed here and everywhere. Why don’t people understand that I am being suffocated by repeated failures and unending deceptions. I am not able to cope and hence trying escape from time to time though with counterproductive effects. Really, disease is not killing me. But I am killed every now and then this leaving death is forcing me into disease and its sequel the final death.” His eyes filled with tears.

He headed in the direction of a bar!

Let us consider another experience.

I had taken my fried to an orthopedic surgeon. My friend was in agony, due to severe soft tissue in inflammation in his leg muscle. After careful examination and discussion with the patient the doctor advised the patient about care to be taken.
I, being aware of the smoking habit of the patient asked the surgeon if it was essential to quit smoking.
To my surprise the surgeon smiled and said, “Already he is in pain and agony. Considering the prose and cons, I would not like to deprive the patient of his small pleasure”
My friend appeared solaced and in deep sense of gratitude for the considerate gesture of the doctor.
After we came out of the clinic, having relieved by the doctor’s permission to smoke, I was expecting him to take out and light a cigarette.
But he did not. On the contrary he gradually quit smoking altogether.

Now let us consider yet another experience!

I had taken my friend to a sex specialist.
Right in the beginning the sex specialist welcome us warm greetings. Then after making us comfortable he just made a remark indicating that sexual disorders do not signify any kind of inferiority and or criminal mentality. Thus first indirectly and then directly suggested to us that we should accept the given situation unconditionally [without guilt and lowliness], love it [i.e. not to despise and resent it] and then respect it [i.e. [ avoid casual approach and take deep interest involve in careful study and conquer it].

Then he began to take detailed history and my friend revealed his heart and all the facts with full confidence.
The sex specialist now examined the patient thoroughly well.
After examination, the doctor discussed several issues such as in laws, children, job, wife’s job, wife’s health status, her perspective or vision of life, hobbies which apparently appeared to be unrelated to sexual problem. Then the doctor wrote in details his prescription and suggested that the whole family visit his clinic for more complete benefits and even more, better quality of life.
Some days later, I met my friend. This time he was with wife and there was that beautiful broad smile on their face. Their eyes were sparkling with happiness and contentment.
Without me asking them any further, they ecstatically exclaimed about the wonderful results.
I requested them to divulge few things if they could without being embarrassed, so that I could use that information for benefit of others.

They said, “The doctor gave us new life. He gave new vision. He gave new dimension. He empowered us to express the best within us for the society and for ourselves. He gave us detailed knowledge about the realm of instincts and their role in life. He later elucidated in details the role of sexual/reproductive instinct, courtship, pregnancy care, parental instinct, rearing the baby, effects of our emotions and thoughts on sexual gratification and how to minimize the damage caused by a variety of stressors on our happiness [which heavily depended on our sexual happiness]. He gave books on stress management and introduced a stress management expert. As a result apart from getting sexual fulfillment [by properly managing the stress] we are conquering blood pressure, diabetes and many ailments related to stress. In fact we have started getting results in terms regaining our youth, vigor and fulfillment in life!
Now I was beginning to understand the meaning of “Accepting unconditionally, loving unconditionally and respecting unconditionally”!
2. Your patients already have a healer within: Invoke him!

Let us go through this letter from my friend. She is a doctor herself.

“Dear Dr
Thank you for your concern about my backache.
Considering your interests, I would like to elaborate a little more on it.
It is true that the backache troubled me a lot. But since I was having a variety of differential diagnoses in my mind, I was afraid that I would have to undergo plethora of investigations and possibly undergo a major surgery! Hence I was reluctant to see a doctor.
But the discomfort went on increasing. Finally I decided to contact a consultant friend of mine. I phoned her. After talking few things, I conveyed to her my problem.
On phone itself she shouted, “How come you have waited so much? It is essential to do ultrasonography urgently. It could be prolapsed disc. It could be lumbar canal stenosis, tuberculosis of bone, endometriosis some other malignancy with secondaries…..”

I was shocked! Hearing to all this all my fear was as if endorsed. I felt as if I already entered some terminal sickness. I was terrified. Chills swept throughout my spine like electric current. I began to shiver with fear and started perspiring. Somehow I finished the phone call. What I was trying to negate had appeared in front of me with menacing face! Now what to do? I was trying to negate all these possibilities because I wanted to some how avoid hospitalization, investigations and surgery. Apart from the thought of expenses, unpleasant atmosphere, doubtful outcome and above all; suffering! Moreover; I had so many preoccupations, obligations and responsibilities to fulfill.
I repented for having made the phone call! But there was nothing I could to then! I did not speak anything to my husband, as I did not want to add to his stress. I thought, he would be shaken by the content of our telephonic conversation.
Somehow I spent that night in extreme anxiety and next day morning I went to a friend’s place. This friend of mine; a simple housewife, immediately reassured me and instantly took me to her family physician.
Since she personally accompanied me, I was feeling greatly relieved. However my anxiety reflected on my face.
The doctor took me in the examination room along with my friend and examined me clinically.
Then the doctor asked few questions about my personal, menstrual and family and past background.
He seemed quite satisfied with my dietary and exercise habits.
Looking at my anxious face with pleasing smile, he affirmed, “Nothing has to be done”.
“But what about the pain? Any investigations? Any internal medicines? Any external applications?”

His face reflected supreme confidence. He repeated with emphasis on every word, “Nothing is required. Continue your exercises and add some yoga techniques, which I would teach you. You would be alright within a week!”
This was a reverse shock! Decompression! Relief! Freedom from tons of burden! I breathed a sigh of huge relief!
I began to thank the doctor profusely. But the doctor seemed to be in bliss for what he done spontaneously and most naturally; and oblivious to my thanks!
Within a week I became all right. I contacted my housewife friend and went back to give the feed back.
The doctor received us with smile and then explained to me that as a result of expectations of the patients and their relatives, expectations of the society, quest for glamour and glitter, desire for power and quick money, fierce competition, various policies, rules and regulations, demands of family, financial liabilities, pharmaceutical propaganda such many factors and most importantly; their own inability to manage the stress; the doctors are under enormous stress. He said, “Doctors are really undergoing enormous turmoil within! Conceptual chaos and value crisis is taking toll of them! They are not able to exercise clinical judgment. Please don’t blame them!”
Looking at my friend’s brightened face he further said, “Your doctor friend has forgotten your clinical wisdom because of stresses and strains in life. Hence she gave lop sided weightage to the differential diagnoses. Hence the fears were compounded!”
Waiting for a while the doctor added, “ Stresses and strains have not only blinded the clinical acumen of doctors, but their ability to see the fountain of strength within themselves and their patients as well! This has made them weak. Weakness perpetuates weakness! I only can be complimented for retaining my clinical acumen and ability to see the healing power within myself and within you. That is why I asked your friend to humbly suggest you a traditional way to invoke and experience the healer within oneself and the others!”!

Dear Dr I feel happy to share with you the joy of relief from not merely backache but regaining my clinical acumen and healer within myself. Now I am able to invokve my patients’ healing abilities in their getting healthier and happier!”

Dr. Shraddha’s story is self-explanatory.

However, Dr. Shraddha has not mentioned two points about her background. I think they will be of benefit to all of us.
First point is: Dr. Shraddha was one of the toppers in her school and college career. She was a not merely intelligent but a very charming sensible girl. However in the course of medical training, she seemed to be getting frustrated. Her curiosity seemed to be suppressed. Her concern for the patient seemed to be laughed at rather than nurtured. Her quest for the happiness of the patients appeared to be treated as in vein and utopian. After entering into practice along with her husband she seemed to be increasingly disillusioned with medical profession. She had begun to indulge and some how cope with. But her melancholy was on the rise. This had begun to get reflected on her physical health. Gradually she had started losing weight, graying, wrinkling, darkening, and getting arthritis and many other complaints. She had begun to lose confidence and had become phobic and feeble. In view of these effects of stress damaging subconsciously, her backache had been only the tip of the iceberg!
The second point: The doctor who treated her effectively had taken care of these points with the help of Dr. Shraddha’s housewife friend [who is trained in Total Stress Management and acted as a support system for Dr. Shraddha]. As a result Dr. Dr. Shraddha is showing beneficial effects on all the organs and organ systems and her over all personality. She is fairer, stronger, confident, quick and agile. She has started looking younger and exudes happiness. From academic point of view Dr. Shraddha has begun to imbibe perspective of holistic health and holistic medicine. Her approach has become eclectic. Her patients get far more options than they were getting earlier. From professional point of view; her practice has multiplied.
In short, her patients, her family and she herself are all happier and healthier!




3. Educational policy [Including medical education] affects the health of millions including you and your patients!

My friend has roaring practice as a pediatrician. Once I asked the secret of her practice.
She said, “It is very difficult to pin point to any particular “secret”, but probably my perspective is working good for me and my patients.”
I asked what she meant by that.
She replied, “In the course of my practice I started realizing that mastering Nelson’s pediatrics is not enough! It is important to study the stress that is playing havoc in the children’s life. I also began to realize how stress affected my own personality. So I decided to study the stress, its dimensions and understand the vast number factors [not described medical text books] affecting health. Having appreciated the importance of these factors; I began to educate the parents and relatives of my little patients about dimensions of stress and how to cope with them. I think it is this understanding that has given human touch to my interactions with my patients and that is working well!”
“Can you give an example of how you educate your patients’ parents and relatives?”
“Yes. I gist of what I teach is as follows.”
“Education is defined in various ways but it can be safely said to have FOUR domains, which are as follows.
The first domain is called AFFECTIVE DOMAIN. This means the state of mind. In simple words affective domain relates to how we feel. Thus when our mind is full of alertness, attention, enthusiasm, buoyancy, affection, concern, joy, tolerance, self esteem, mutual respect, mutual trust, commitment, dedication, confidence, positive and victorious spirit, we would call it healthy affective domain. In addition the zeal and concentration needed in the pursuit of excellence in intellectual field, tenacity and endurance required in skillful activities and patience and commitment essential for “satisfying and socially beneficial actions” constitute affective domain. The purpose of education is to nurture this domain by designing suitable curricula and syllabi.
The second domain of education is called PSYCHOMOTOR DOMAIN. This implies ability to appreciate skills and ability to perform physical and mental skills, with speed, accuracy, elegance, ease of performance etc. This may involve appreciation and performance of skills such as surgery, playing a musical instrument, playing basketball or doing carpentry! The purpose of education is to nurture this domain through designing suitable curriculums, syllabuses; but also by providing sufficient practical and demonstration classes with all the necessary equipment.
The third domain is called COGNITIVE DOMAIN. Cognitive domain incorporates accurate perspective, contemplation, correct perception understanding, conceptualization, analysis and recall of problems, ability to evaluate, synthesize, correlate and make decisions, make appropriate policies, plans and expertise in the management, administration, etc.
It is clear that all the domains have three components viz. cognition [Perception], affect [Feelings] and conation [Response].
And fourth domain is productive domain!
Let us consider the traditional system (in India) in a dispassionate manner and scrutinize the merits and demerits. It is obvious that we will not be able to consider all the details of the system as they varied from time to time and from place to place. But a general review of the system would help us to rectify the education system! This can be done only if we rise above the petty considerations of religion, region, caste, political power and other vested interests. This would help us rectify the present education system.
Conversely, unless the existing education system is rectified appropriately subsequent generations may not be able to develop the four domains of education adequately and hence won’t be able to be healthy enough.
Many of you may argue here that this task cannot be accomplished by common people, but can be successfully accomplished only by the politicians, political advisors, the decision-makers and the top administrators.
This argument is partially valid, but not completely. It has to be appreciated that no statesman, no political leader, no policy maker and no administrator can bring about change in an existing system unless, there is consensus about these changes in the vast majority of people whose cooperation in such matters is very vital.
So let us take at least a cursory look at the traditional system of education in India.
The traditional education system in general ensured that:
a] Careers were not selected on the basis of monetary gains,
b] Careers were not selected arbitrarily on the basis of idiosyncrasies and whims,
c] Some lucrative careers could not be sought after, in preference to the others,
d] All careers ensured income and production from early age,
e] All careers ensured that society was benefited,
f] All careers ensured security to all the social groups,
g] All the careers ensured intimacy and closeness between young and old in the families.
h] All careers ensured ethical education and passage of experience from generation to generation.
In my view these were merits.
But it is probably true but not entirely clear if
A] The traditional system was marked by deprivation of scholastic education to many people,
B] It was also marked by apparently unjustifiable distribution of a variety of jobs amongst different castes.
C] It did not have the infrastructure for collective scientific and technological efforts.
D] It suppressed the talents and individual aspirations amongst the children and parents from many castes.
E] It had an element of arbitrary imposition of hierarchy and exploitation.
As the education shifted from homes, home industries and farms to; nurseries, K.G. schools, schools, colleges, universities, corporate industries, research institutions etc.
Cognitive domain suffered because of:
a] Huge number of students, in a single class making following three things almost impossible. These things are i] individual attention ii] dialogue iii] discussions,
b] Lack of adequate salary, accountability, incentive and economic security to the teachers taking away the initiative of nurturing cognitive domain
c] Increase in alienation with respect to student’s background and aptitude
d] Lack of adequate incentive to the students in the form of creativity, production and earning, service to the family and service to the nation [thereby thwarting motivation required for building up cognitive domain]
e] Lack of conviction essential in the growth of cognitive domain in the teachers and students because of outdated practical and demonstration classes, lack of interdisciplinary dialogue and in general the irrelevance of education to the realities of day to day life in as much as almost predictable consecutive unemployment at the end! The lack of conviction could be partly due to lack of participation by teachers in decision-making, policy making, development of curricula, syllabi etc.
f] Emphasis on recall and hence rote learning thereby denying free inquiry, reading, questioning etc. thereby directly thwarting the cognitive domain
g]] Too many examinations with irrelevant parameters or criteria of evaluation [besides being unfair in many instances] leading to misguided and in most cases counterproductive efforts thus adversely affecting the cognitive domain
h] Competitions where the manipulative skills, callousness, selfishness are given more respect, [thereby destroying the enthusiasm of growing in cognitive domain]
i] Information explosion affecting cognitive domain by either causing enormous and unnecessary burden on memory or inferiority complex
j] Pressure of interviews causing constant tension and sense of inadequacy, right from the tender age,
k] Protracted hours of homework in schools denying the students their legitimate right to enjoy their childhood [causing hypokinetic stress, denial of parental rearing] and make them physically, mentally and intellectually unfit to grow in cognitive domain
l] Irrelevant and unnecessary information loading in lectures in the form of monologue, leading to suppression of the spontaneity, originality, interest and enthusiasm so much required in cognitive development amongst the students,
Affective domain suffered due to,
A] Isolation of the children from their parents and their domestic environment at an early age [Making the parents also equally sad]
B] Lack of warm bonds due to huge number,
C] Cut throat competition,
D] Inadequate facilities of sports, trekking, educational tours, recreation and physical development etc
E] Alienation from one’s social environment and culture
Psychomotor domain suffered due to
A] Almost total lack of opportunities to actually participate in skillful activities such as drawing, painting, sewing, sculpturing, carpentry, knitting, weaving, music, agriculture, horticulture, other handicrafts, various sports, performing arts etc.
It is important to realize that promotion of psychomotor domain is evident but in its caricature form. It has no concrete economic realistic basis. The activities have no economic incentive and no productive element.

The present education system basically and almost completely prevents a huge section of society such as teachers, students, clerks, servants, sweepers and many others such as education inspectors, etc. from being creative and productive. In addition it causes colossal loss of space, electricity, construction cost and so on. In addition because of the typical emphasis on rote learning it leads to phenomenal waste of educational material such as paper, bags, pencils, ball pens etc.
Lack of productive element in education not only causes colossal loss to nation but it also causes economic loss to children while suppressing and starving their psychomotor domain! The lack of productive element and economic incentive is a single most important cause of
1] Reduction in the dignity of labor amongst those who continue to learn, as well as reduction in the income of the concerned families and the nation
2] Lack of education, lack of employment and starvation or criminalization amongst those who are forced to drop out because of poverty [Note that children in villages normally contribute to the earning of the family].
3] Inhuman suffering of those dropouts, who somehow manage to get into cheap labor for subsistence.
It has to be appreciated that billions of rupees are spent on construction, decoration and maintenance of schools and colleges. Billions more are spent on payment of millions of teachers and other staff members engaged in unproductive exercises. Billions are spent on electricity, and so called educational material. Billions more are spent on the exams conducted to test the “capacity and merit of rote learning”. This way we weaken the national economy, jeopardize the developmental activities and force millions of students to drop out due to economic reasons and get into the hell of child labor, besides starving and suppressing the cognitive, affective and psychomotor domains of millions who are in schools. In short, present day education system harnesses [amongst those who continue to learn] arrogance coupled with lack of confidence leaving all the domains viz. cognitive, psychomotor and affective, [including creative and productive skills and physical health] defective, deficient and underdeveloped. Further, when this education fails to give a job, it tends to create vindictive attitude transforming an individual into a criminal or develops frustration and transforms an individual into a mental wreck.
It must be appreciated that some institutions and individuals are making illustrious efforts in the direction of rectifying the education at their level.
But the mainstream education system has to ensure that the student must have economic incentive for what he/she is privileged to make. Besides, everybody connected with education directly or indirectly must be involved in production or service.
Everyday; approximately
20 % of the time must be spent in production of suitable items and service can involve community projects such plantation, cleanliness etc.
20 % of the time must be spent in physical activities such as sports, exercise, trekking, hiking etc.
20 % of the time must be spent in personality development i.e. broadening of perspective through various means such as invited guest lectures, seminars, discussions on holistic health, educational tours and visits to places where the student gets exposed to rapid developments in the society such as laboratories, airports, government offices, share market, farms etc
20 % of the time must be spent in entertainment such as playing musical instruments, dance, painting or anything that makes a student happy such as mimicry, singing, story telling, drama, movie etc]
20 % of the time must be spent on cognitive domain i.e. study of languages, history, geography, mathematics etc with utmost emphasis on interpretation and relevance in day to day life. Thus typical questions in the examination of history, languages should be totally done away with. The subject such as economics, psychology, civics, philosophy, logic, sociology etc must include field work and made relevant to the present society”.
Finally I conclude by saying, “If we are able to rectify the education system (not only in India but all over the world including medical education), then hypo kinetic stress, stress resulting from rote learning, undue tension of the examination success, cancerous growth of copying, corruption for getting grant, and consequent stress can be prevented. Indeed, drop outs from school, child labor, unemployment and corrupt practices affect our health and cannot be ignored by anyone aspiring for holistic health i.e. global welfare!”
I am sure you saw in the above instance; how you can make the life worthwhile and truly successful!

4. Take care of yourself!

There is a glaring example of a young doctor!
He is a physician. He stared practice and by virtue of his merits started getting good work. His income started swelling! His market demand also grew. Pharmaceutical companies started sponsoring him for conferences. Gifts started pouring in. His busy practice started keeping him awake at late nights.
He started getting hardly any time for his family. The communication amongst the family members began to dwindle. Initially his wife and children complained. But he did not give much importance to it. He thought it was more important to secure them by earning more money. Gradually his wife and children developed distance. They seemed to stop needing his company. They seemed to; no more require dialogue with him.
It was after some years that he sensed this change. Those dry expressions, those lusterless eyes and those curt words struck him.
He realized that something was going wrong somewhere.
He went to one of his old teachers. He narrated these things and sought his advice.
His teacher carefully listened to his grievances. In fact the grievances of his family members! He could see that the strained relationships visibly disturbed the young doctor. But; what more the teacher noticed; was deterioration of the physical health his past student!
The teacher (himself a doctor; practicing holistic medicine) had prepared a long questionnaire for practicing doctors. The questions included detailed clinical history, personal history, family history, occupational history and so on. More importantly it included questions regarding goals, aspirations, purpose and ambitions in life.
This young doctor began to think about these questions! It was not easy to come to any conclusion, as there were no readymade answers!
After filling the whole questionnaire, the young doctor admitted frankly that he was at a loss to give satisfactory answers to questions such as goal in life!
Now the teacher said, “Don’t worry. The questionnaire is not meant for filling; completely and correctly. It is meant for making you aware of hitherto concealed aspects of your personality. Now that you have made attempt to fill it; let us discuss further and come to some conclusions.
After having protracted discussion the young doctor realized that every one of us has body and hence physical needs, instincts and instinctual needs, emotions and emotional needs, intelligence and intellectual needs and conscience and spiritual needs!
These needs give satisfaction in ascending order, that lasts for longer time and that can be distributed to larger number of people!
On this background when he began to evaluate his professional practice, he realized that he grossly underestimated his spiritual, intellectual, emotional, instinctual and even physical needs! He realized that amongst these various needs he cared for physical needs to some extent! Market forces, media hype and illusory financial aspirations to fulfill phantom physical needs of future were dragging him! By getting engrossed in the practice his ego and hence emotions also were gratified to a certain extent.
He now decided to give justice to his conscience with utmost priority. He put the intellectual, emotional, instinctual and physical needs in that order.
He realized that perspective of global welfare adds new dimension and power to one’s being. He realized that such perspective is a nurturing nectar or ambrosia of life! It makes one happy and also emanates healing happiness!
He realized that conscientious intellectual pursuit [and not obsessive one] adds holistic perspective and hence clinical accuracy and therapeutic efficiency. Moreover he grasped the protective importance of intelligence in the quest of justifying conscience and fulfilling emotional, instinctual and physical needs!

Now he had the way to find out what could give him maximum happiness, that would last for maximum time and that could be distributed to maximum people.
He reorganized his practice on this basis.
He reaffirmed to himself that he would consciously get completely committed to the total welfare of the patient.
He began to take keen interest in every patient irrespective his personal likes and dislikes. This enabled him to develop heart-to-heart communication with every patient. Every patient became his friend and family member. This prompted him for active search for alternative modalities of treatment, which could suit a particular individual patient. He started giving more time to his patients [by not getting obsessed to see more patients in less time and get more fees]. His clinic became a source of peace and hope for the patients.
He stopped working at specific time of the day [by resisting the temptation to get more money] thus returning home in time. He began to take interest in the rearing of the children and the interests of all the family members. This development was welcomed by one and all!
Everyone got reunited with the bond of love and care. The smiles returned! He realized what he had missed and how valuable it was!
The husband and wife once again began to share optimum time and strengthened each other’s convictions.
Lastly, he began regular walk, exercise, yoga, and swimming and involved his wife and children in those activities. He paid scrupulous attention to diet, clothes, environment and such varied factors that could satisfy physical needs appropriately.

After few days he met his teacher again.
He asked the young doctor, “Are you satisfied with your progress?”
The young doctor was still hesitant! He was not sure!
The teacher smiled and said, “You are on right track. You here onwards, have to reach the top decision makers. You have to influence them beneficially for the welfare of millions through proper policies and decisions. This would satisfy your spiritual domain and give you fulfillment. For this; all of you need to practice and propagate NMASMARAN”.
The young doctor prostrated in front of the wise teacher and returned home.
Today this young doctor is not only being paid his fat fees with great respect and gratitude but doing very well on every front! What is more, he is beginning to get into the position of opinion maker!


5. Teach them win-win formula

Who is a successful practitioner? One who is paid more? One who has more crowds? One who works more?
You must have seen doctors who are paid more, but because; one has to pay them. Even as some patients can easily afford; in most instances payment is made reluctantly. One may agree that such doctors are usually sought after because of their expertise, lack of other options and practical need. But what about fondness and love?
Some doctors are very busy. Their clinics are always crowded. If you look into the reasons, you may find that they are inexpensive and easily accessible. Thus here is also compulsion, financial compulsion. Some times these doctors are “tried” as first step.
Some doctors work more. They are dedicated. They may get genuinely faithful patients, but; what about their families? What about their own health? What about the time they should be spending in academic pursuit?
I don’t want to blame any of these categories of doctors.
But consider this example!
Here is a neurosurgeon. He is in his clinic at right time. He is fresh and enthusiastic. His staff members welcome his arrival with joy and enjoy his presence. He is confident and modest. He is dignified and respects the dignity of his patients, especially lady patients. The patients are brightened by his reassuring gestures.
He listens carefully and performs the clinical examination meticulously. He makes decision that suits the patients maximally. He is open-minded and keeps as many options as possible.
He is methodical and always ready to do any work at any given moment.
His staff members are always ready to help him. They feel privileged to work with him.
Apart from these qualities, he is extremely open-minded and ready to learn from even his juniors! He is always helpful and ready to teach his junior colleagues.
He is never seen showing tantrums at the time of surgery. He does not lose temper. His nurses are relaxed and alert at the time of surgery. He has trained them very well.
Apart from all this; there is no grumbling or backbiting. He is never heard gossiping. When he returns home he has all the satisfaction on his face.
What is most important, everyone feels victorious with every success of his, be it surgery, be it guest lecture, be it winning an award, be it publication of paper!
You would agree with me, that he/she is a successful practitioner, whom his/her patients, colleagues, associates and everyone related to him/her love. In other words, when people around you participate in your success or failure; as if it is their own; then that is success…. rather great success!
It is very essential therefore to learn and also to explain the patients and their relatives, that three formulas should be shunned!
The first formula to be discarded is win-lose formula.
In this formula if the doctor succeeds, then he would feel triumphant. He would take all the credit. The patient’s relatives would prostrate [or would be expected to prostrate] as “losers”.
The doctor’s head would be bloated with pride and arrogance and patients head would droop down in meekness.
Now the second formula to be discarded is lose-win formula.
Here the doctor loses. He declares his inability to handle the patient or the doctor’s efforts fail and the patient succumbs.
The patient’s relatives having no trust and sense of cooperation; would develop contempt and hatred about the doctor as he is loser and patient’s relatives would get excited aggressive and doctor would go defensive.
The third formula that needs to be rejected is lose-lose formula.
This is a strange formula where the both, doctor as well as patient and/or patient’s relative would act cynically and apathetically. Neither the doctor, nor the patient/patient’s relatives would aim at getting well. Together they accept the defeat without struggle; as if determined to lose!
Now the fourth formula to be taught and adopted in every day life is win-win formula.
In this win-win formula, patient’s recovery is considered to be the success of the patient, patient’s relatives as well as the doctor and his team and all those involved directly and indirectly including the manufacturers of the drugs! When the doctor, the patient and the patient’s relatives work together to win together, they usually win! Not only they win but also they celebrate it as their common success!
The win-win formula is not only important for becoming successful as a doctor, but it also assures wellness cycle, in your life and in the life of every one around you.
Remember, all other formulas always throw us in distress cycle. They result from as well as lead to anxiety, violence, abusiveness, melancholy, cynicism, and lack of appreciation, disinterestedness, apathy and distrust in spouse, child, friend, relatives and destroy one another.

6. Time management: a way to greater efficiency
“Three score summers when they are gone, will appear as one!”
“Time flies”
“We listen to people saying, “If I had some time I would have been able accomplish this!”
It is indeed extremely important to use time in a wise way! The wise use of time assures living to maximum satisfaction!
Sometimes people are seen asking their more successful friend about how does he manage so many things? How does he get time for it?
They do not realize that everybody has twenty-four hours of the day! No body gets extra time!
Consider these points in time management and judge for yourself if you could extract more satisfaction and greater success!
We should be cautious about our time but being harsh disciplinarian usually brings more pain than happiness! Being perfectionists also usually brings more disappointment than satisfaction! Time management is important but expecting others to comply with your standards usually leads to dismay and bitterness. Time is extremely precious. But getting obsessed by time leads us nowhere. It can turn you inhuman. It can take away the humane and warm moments in your life! It can deprive you of the excitement in life. Another point about time management is taking the burden about the future! The thought of “better time management” in future obsesses us! Avoid it!
How are you going to judge your success in time management? Only financial yardsticks should not evaluate time management!
When it comes to social interactions, people do expect us to spend our time for them. It is important to assert our priorities in such instances. We must learn to give due value to our time and alertly protect it from being “stolen”!
This is especially true if you are sensitive, vulnerable and malleable!
The other side of the coin is yearning for others’ company and time! Be particular about not stealing other’s time as that person may despise you in future.
Time management involves keeping appointments meticulously. Always keep your appointments as that may involve time of many individuals [e.g. meetings]
It is rather preferable to make a habit of asking, “Am I doing at present, what I prefer most?” Let the future take its own course! Do not worry!
One of the most important ways of time management is to determine the sequence of priority.
First find out what is most important for you by using the formula viz. what gives you maximum happiness, for maximum time and that can be distributed to maximum people. After you determine what is most important for you, give it top priority. This is called prioritization. After you prioritize it, assert yourself by actually doing it.
After you assert it you may like to repeat and reinforce your satisfaction or you like to revise and improve upon your performance. According to this method, you generally succeed in fulfilling the most important task and never regret for having forgotten something vitally important.
Another important point in time management is using sense of proportion.
Many of us spend exorbitant time to save small amount of money. In the quest of getting better bargain we go from shop to shop. We tend to forget how valuable time is being wasted in the process. The sense of proportion has to be exercised while planning to buy certain commodities. We tend to get bogged down by our budgetary provisions and keep postponing purchasing of certain commodities that would give us great deal of comfort. It is important to realize how much time of our life is going waste without the enjoyment of those commodities. It is important to make a quick and courageous decision to purchase commodities, which would improve the quality of our lives. Another aspect of sense of proportion is waiting. Many of us tend to wait for a bus to save money on taxi. We must realize that we are depriving ourselves of precious time that we could happily spend with spouse, children or friends! This sense of proportion also comes into play when it comes to appointing housemaid, driver, assistant or other helpers. We must learn to appreciate how much time we should be saving to spend in more useful activities rather than worrying about relatively less precious money of salaries!
The other way of time management is using technology, human resources and other tricks.
Thus, you can use telephone, SMS, email, Internet, printed material, compact discs, and videocassettes for effective communication with large number of people in small time. Similarly you can achieve education of large number of patients in short time. This saves your time tremendously. Talking to every patient can be wasteful and can be tiring and irritating for you and your patients and even their relatives!
Another way of time management is to delegate your work to suitable associates. But this requires that you train your associates properly. Training includes, inspiring, motivating and involving them in your mission and also training them in technical matters. If you are able to do this then your work force, work output and satisfaction all multiply.
Yet another trick is to do more than one thing at a time!
For example, when you take morning walk, practice NAMASMARAN i.e. remember the name of God [if you don’t strongly disbelieve in God] and simultaneously harmonize abdominal breathing with your steps.
When you watch news on T.V., use techniques of reflexology or acupressure, or do head massage. Head massage can ensure comfort to your head, improve your sleep, improve the health your hair and also get exercise to your hands and shoulders.
Elderly or diabetic individuals can utilize the time of T.V. watching by doing foot massage.
You can perform foot exercises, leg exercise, knee exercises or practice suitable yogasana also while watching T.V. One can take food or a cup of tea as well.
One can take specific exercises of shoulders, neck [BRAHMA MUDRA], back while watching T.V.
One can practice some techniques such as Bhramari, Kapalbhati, Ashvini Mudra, Bate’s technique, Anulom Vilom while traveling (and not driving) in bus, train, plane or car.
Many of you may be already practicing this trick. But what is important is now you can appreciate its importance even more!

Having discussed this much last for steps can be simply stated!
7. Simple and straight forward? No! profound and wise!
8. Discuss with pharmaceuticals the perspective of global welfare, which is a treasure of lifetime!
9. Do not get imposed and donot impose yourself.
10 Be a leader and not a follower!

Music, dances, sports, art and everything for that matter can be; and are sold; as stress management
techniques! Perfumes, shopping, spas, colors, designs and a variety of appliances are promoted widely as stress management techniques.

The question is whether these techniques are relieving our stress or no?

To share a personal secret with you, my experience is (and it may not apply to every one); every sensible technique has some benefit but total stress management is not possible with any of
these (in absence of NAMASMARAN)!
Because they themselves can not reach to the roots of stress!

At least so far as I am conscious and alert and not intoxicated and dehumanized, it is not possible to claim successful stress management merely by these techniques (by neglecting the causes of universal stress and its disastrous impact on mankind)!

It is for you to verify if I am making sense!

How to correlate the concept of stress
management with the suffering and sacrifices of individuals for others? Is the suffering for others is opposed to STRESS MANAGEMENT?

Does “stress management” mean “being mean and happy?” Does it mean that the suffering of great or saintly personalities has no place in stress management?

My answer is:
The great individuals
A] could not be truly comfortable in the usual indulgences. They always were obsessed with the welfare of mankind. They might differ in their vision and solutions, but they were forerunners of Total Stress Management!

B] When I studied the stress from physiological, patho-physiological and holistic point of view, I thought that we are stressed by millions of stressors including; poverty, ignorance, crimes, perversions, diseases and misery of every kind. But the common denominator in all these is PETTINESS!

C] I thought practice of NAMASMARAN could bring about spiritual blossoming from pettiness by transforming us from within and thereby transform our perspective, policies and plans so as to unite and tangibly transform the whole world!

This concept and the question “Is Stress Management Possible?” are open for verification and investigation respectively!
