The prevalence of blindness among children varies between 0.2 /1000 to 1.5/1000 with a global estimate of 0.7/1000. This means that there are about 1.4 million blind children worldwide. It has been said that a child goes blind every minute. Children who are blind have to live a life of emotional, social and economic difficulties which affect the child, family and society.

However it is worthwhile to note that nearly 50% of the 1.4 million children blind worldwide are needlessly blind. Most of the causes of blindness are preventable. Refractive errors are easy to treat with simple prescription of glasses. Vision needs to be checked for all children by the age of 3 years by a qualified health care professional as the child will never express that it cannot see well. Other important causes of preventable blindness include “amblyopia” or lazy eye, squint and childhood cataract.

When there is a difference in refractive error between 2 eyes or the eyes have squint or the visual axis is blocked by a cataract or any white opacity, the eye does not see and the brain recognises the image from the better eye and ignores the image from the poorly-seeing eye. The weaker eye is called the laze eye. In the early stages this may be reversible, however as the child grows older this becomes more and more difficult to reverse. Early detection is essential. It is important to recognise this condition as this is very common and affects approximately 3 of every 100 children. Vision screening is essential to diagnose this condition as the eyes will appear normal. With prompt treatment a reversal is possible though in many cases beyond the age of 6 years the condition may not be completely curable. The reason is the vision pathways develop faster in childhood and their development is complete by about 8 -10 years of age. Simple home exercises like patching the good eye are prescribed or sometimes only eyedrops are administered. It is important that the child is under the regular care of an eye specialist to monitor the vision in the eyes. Parents and guardians have a vital role to play in the treatment of this condition. The treatment may be needed for a few months till the vision is completely improved and needs patience and perseverance. Recent research has shown that amblyopia or lazy eye can be cured even when the child is older.

Squint is a misalignment of the eyes so that both the eyes do not focus on the same point of interest. This deviation results from the failure of the eye muscles to work together. When a child is born the eyes may not be able to focus on objects. But by the age of 3 months, an infant’s eyes should have the ability to focus on small objects and the eyes should be straight. A six month old infant should be able to focus on both distant and near objects. It is important to detect and correct squint early as an uncorrected squint may lead to a lazy eye. A person with squint can use only one eye at a time whereas a person with properly aligned eyes will be able to use both eyes to judge distance and depth more accurately. Certain careers need good binocular vision. Squint can also affect the appearance and self confidence of a child. A squint is easy to spot. One eye maybe turned towards or away from the nose. But small squints may not be seen easily and may need an eye specialist to pick up these squints.

While in most cases the cause of squint is not obvious, various conditions like refractive errors, focussing problems, nerve injuries and poor sight due to a cataract can lead to squint. Sometimes squints run in families. A child with squint needs a full eye check up. Young children are uncooperative and sometimes this may mean need for examination under sedation. Treatment often involves spectacles to correct the refractive error and the glasses need to be worn most of the day. Lazy eye if present may need treatment as well with patching. Surgery to straighten the eyes may be needed not only to improve the appearance but also to ensure the child uses both eyes together. Surgery is straightforward and involves tightening or loosening the muscles. Surgery is very safe. Modern techniques ensure recovery in a day and resumption of activities the next day. The success of squint surgery is very high and frequent follow up is not required.

Childhood cataract comprises about 14% of preventable childhood blindness. Cataract refers to cloudiness of the normally clear lens. Contrary to popular belief, cataracts can occur in children. Cataracts can cause decreased vision by preventing the light rays from reaching the eye and can lead to a lazy eye or amblyopia which can be irreversible. While in most cases the cause of the cataract is unknown, some cases can occur in conjunction with a genetic or metabolic abnormality. Amongst the preventable causes of cataract, rubella induced cataract can be easily prevented with appropriate vaccination. An estimated 2,38,000 children are born with congenital rubella syndrome every year. In south India, about 25 % congenital cataracts are known to be associated with rubella. Rubella immunization of women of the child bearing age can prevent congenital rubella syndrome. Some forms of cataract are genetic and occur following marriages within families. Early identification is essential. Recognition of the cataract is usually by a family member or teacher as the child is not going to express that he or she has cataract. A whitish opacity may be apparent in the child’s eye. The child’s eye learns to see only when clear images get focussed in the eye. If this does not happen the child’s vision will not develop normally.

Urgent surgery is usually required followed by the use of glasses or contact lens. Surgery is not as straightforward as in adults and the child’s eye behaves differently from adults’ eyes. Recent advances in surgical techniques and lenses which can be implanted in the eye, have made the surgery quite safe and successful. The parents are as involved as the eye health professionals in the rehabilitation of the child. With specialised services and modern technology, the outcome of surgery has improved. Early recognition, early intervention and early rehabilitation will increase the probability of good visual results.

A child may appear normal and yet have a visual problem. In most cases the child is unable to convey the problem and hence the eye diseases can go undetected. Hence it is our duty to ensure that all children are examined at least once by a qualified eye health professional, before the third birthday.

It is said that “intellectual strength is more important than just physical strength”. This is all time true statement. For significant achievement in any field, Intellectual performance is rather most important tool and there could be hardly anybody who will disagree to this. Every parent dreams about best scoring of their child in academics in all the fields. There are end numbers of products available in the market today claiming such therapeutic benefits and also new products are coming up every other day. It creates confusion amongst consumers as to which products are genuine and which are not. The advertisements are also rivalling and hence it is essential to identify the appropriate one. Besides this, what are the other treatment options, what are the correct doses, how these formulations work, how is the knowledge stored, how does the recall process work, what diet is good for nourishing the activity of brain, importance of diet, importance of prayers, significance of exercise and rest? Here you will find answers to all such questions and their scientific basis.
Three steps: While during common cold, blockage of olfactory nerve there is a total loss of sensation of smell. This is very common example experienced by most of the people. Similarly, if there is any kind of blockage in the optic nerve, the sensation of visual experience will be blocked; auditory nerve will result in impaired listening etc. For acquiring the knowledge, the body uses 5 sense organs. Ears, Skin, Eyes, Tongue and Nose are collectively called sense organs. The sensations experienced by these sense organs are transmitted to the brain. The sensory nerves conduct messages through chemical transmission and then they are stored in the brain. Unless the knowledge is stored properly, one can not use it at any point of time. To explain this concept, I wish to give one example. An infant, who is deaf since birth, remains dumb throughout the life. His vocal organs may be absolutely normal despite he could not process the knowledge of speaking which is essentially linked with hearing. Hence he or she will never know “what is word and how to pronounce it”. The prerequisite for uninterrupted transmission of these senses is to have the healthy state of the nerves and also they should be properly lubricated. The myelin sheath of nerves is made of about 80% lipid and about 20% protein. Therefore it is essential to maintain the lubrication by way of providing adequate unctuous vehicle. Proper lubrication to the sense organs help to improve the oxygenated blood supply and thereby help to establish the physiological functioning of specific sense organ. Applying a drop of oil or any other lubricant to finger facilitates inserting the odd-sized ring easily. This is an easy example to understand the importance in relation to the grasping ability of the sense organs. Oil and ghee are the main natural lubricants available since time immemorial. Ayurveda has described the properties of cow-ghee as follows:

It means that cow ghee improves grasping, storing and recall, the three aspects of brain function. It also stimulates appetite, improves quality of life, physical endurance, vigour and eyesight. Nose is the gateway of brain and any product introduced through the nostril has immediate access to the brain tissue. Even modern medicine has identified the potential of this path which bypasses the blood brain barrier and without much efforts, the valuable drug can be made bio-available in plasma and CSF in just 1.5 minutes. Use of nasal insulin is a well-known example to understand this phenomenon. Recently there is also nasal spray for calcium supplementation. Here is the first step towards improving the grasping capacity of the brain which mentions administration of ghee based product through nostrils. Ayurveda has described this mode of administration 5000 years ago by the name of ‘Nasya’ treatment. Besides cow-ghee, some valuable herbs like saffron help to impart phenomenal antioxidant effects on brain cells. It is scientifically proved and possesses a strong supporting reference. Nose is directly connected to the brain through olfactory apparatus; it is connected to ears through Eustachian tube, to eyes through lachrymal ducts. Therefore nasal treatment helps to restore the physiological functions of all these organs and nourish the brain’s grasping capacity. Ayurveda recommends 4-4 drops in each nostril daily to achieve the said therapeutic benefit. Despite improvement in intellectual competence, unknowingly this treatment imparts lots of other advantages like prevention of recurrent attacks of common cold, arrests hair fall, overcomes fatigue of eyes due to excess-reading, combats burning of eyes, protects occasional blocking of ears, protects vision defects etc. The ideal time to administer these nasal drops is early morning or late evening after the sun-set. One should lie-down for 5 minutes afterwards. Rarely the feeling of the drops coming into throat is noticed which may be ignored or one can have a sip of warm water. This nasal medication helps to speed up the grasping process of knowledge without any difficulty. A readymade preparation by the name of Cleverin nasal drops has been recently developed and approved by the Food and Drug administration which possess multiple therapeutic benefits attributed to grasping of knowledge by empowering the neurotransmission by cleansing and lubricating the sense organs.
Second Step: After grasping, the second step of improving the intellectual process is of increasing the storage capacity. The knowledge acquired has to be stored properly which can be utilised later as and when required. It is scientifically proved that the active constituents from herbs like Brahmi, Shankhapushpi, Shatavari, Ashwagandha helps in improving the intellectual performance by different mode of action. Some help by way of enhancement of protein kinase activity, Increase in protein in hippocampus, alleviate stress and help to minimize the release of stress hormone, some have neuro-regenerative activity, some help by improving the oxygenated blood supply to the brain and some prevent oxidative damage by imparting anti-oxidant activity. In short these herbs play a major role to improve the storage capacity of the brain. It is as simple to understand that if more books are ordered in the library, it becomes essential to make additional arrangement of accommodating them by adding number of shelves. Most of the marketed Ayurvedic and herbal formulations work on this principle. The activity starts in about 2 weeks. Nobody can deny the fact that memory related functions can’t be measured like body temperature. Even then their efficacy remains unquestionable as huge amount of data is available across the world and also the safety parameters are well established. The important aspect is to know the quantity of each item in the formulation. If inadequate quantities are consumed, one can not expect the therapeutic benefits in desired time. Besides improvement in intellectual abilities, these herbs also possess several other benefits like improvement in physical endurance, improves hemoglobin level, keep RBC, WBC in right shape, improves the immunity, hair melanin, prevent calcium depletion, and delay ageing process. Recently German researchers found they could use specific perfume at night to re-activate new memories in the brains of students during sleep and the volunteers remembered better on the next day. The test conducted showed the 97 % result and the control group performed only 84 %. During the studies, fMRI scan showed that the activity of Hippocampus was stimulated during inhalation of particular odour. Based on this research, a novel product in the form of Dhoop stick is prepared. This Cleveroma stick does not contain strong perfumes but it is composed by Ayurvedic aromatic herbs like Tulsi, Jatamansi etc. which improve the recall function of the brain. Students should light this stick in the bed room where they intend to sleep because the activity of hippocampus is at its best during sleep.
Prayers have scientific base: Prayers towards God and offering respect to seniors helps to strengthen the brain’s overall performance. This statement can be scientifically proved here. To understand this, we must match up few things together. It is an established fact that cortisol hormone is released due to stress or fear. This is therefore called stress hormone. Cortisol causes serious damage to the brain. Hence in acute and serious situations an individual becomes insane. He or she could not make any logical decision. Second situation when a child is thrown up, he laughs as he enjoys it. The confidence level of the child is to the extent that he is 100% sure about his security. Therefore in a situation where he should be afraid, he rather takes pleasure out of it. The hormone cortisol is not released and the brain remains away from its ill effects. A prayer towards God and offering respect to the seniors gradually builds the confidence level that a person feels enormous support. In olden days, it was a tradition but has a scientific base to offer respect to seniors and pray God before leaving for long journey. As there were no vehicles and even roads, people used to travel in either bullock-cart or ride on horses. Getting food during travel, climatic conditions, and possibility of attack from wild animals were major hurdles and only blessings used to be the moral support. The customs were simply followed but the hidden treasure is now understood by the modern world.
The purpose of lighting candle or oiled light during prayer also has a scientific implication as it plays a role of Tratak (a yogic practice for improving concentration). Lighting of Agarbatti and the aroma helps to improve the blood circulation to hippocampus which plays key role in recall process of the acquired knowledge. There are lots of Mantras to perform during prayers and the aroma helps to rehearse them in proper order and manner.
Importance of regular exercise: How much exercise you do is not important but how regular you perform it is rather more important. A 76 years old man reached the wedding hall slightly late for his own wedding is a fact known to the author. This was because he did not want to compromise with his exercise schedule. No doubt those taking rigorous exercise look well-built but, most of the times, their immunity is not up to mark. Their bones are also brittle and get fracture even with a small trauma. It is also found that such well-built individuals do not have an issue. This is because the optimum nutrition is utilized by the muscular portion and other systems remain under-nourished. Therefore is good to perform little but regular exercise. Apparently such people would not look as attractive in physical built, but their overall physical and intellectual abilities will be far better than a robust individual.
Intellectual exercise: As regular exercise can build good muscle power, same is the case with brain tissue. The more you keep them functioning, their performance will improve to the top. In the course of exercise, the muscular action is repeated in the same sequence many times which tones-up the muscle fibers. The brain also achieves improved ability by repetitively doing the same task. Forgetting is a natural process in living creatures. Man forgets more because his mind is diverted to too many subjects throughout the day. Intellectual performance can be further improved by meditation as it tones up the brain to stick to one subject and not to get diverted. Very few people remember the dreams on the next day. 6. give them proper nutritional diet in their daily dieteric habbit like fruits, fruit juices, raw vegetables in the form of salads, green leafy vegetables as much as they can have it.
7. avoid mixing up two thermals at a time like heat and cold alternately in a particular time as for eg. Going directly to sun exposure immediately after having cold bath. Will affect kids and will develop allergic respiratory problems.

SOME OF THE HOMEOPATHIC MEDICINES FOR RESPIRATORY INFECTIONS

1. ACONITE – hoarse, dry croupy cough, labored breathing, shortness of breath, larynx sensitive cough worse at night and after midnight. Red dry constricted throat with numb, prickling, burning, stinging pain inside. Tonsils swollen and dry. Allergic rhinitis with runny watery discharge and sneezing and nasal blockage is there. Fever with all the respiratory complains
2. BARYTA CARB – very peculiar medicine for recurrent tonsillitis and throat infections. In any case of tonsillitis this is the first medicine to prescribe. Who catches cold very easily and got throat infections so frequently with total loss of voice due to swelling of tonsils.

6. GELSEMIUM – dryness of nasal fossae. Swelling of turbinates. Watery excoriating discharge. Acute coryza with dull headache and fever. Slowness of breathing, with great prostration. Oppression about chest. Dry cough, wit sore chest and fluent coryza. Aphonia, acute bronchitis, respiration quickened, spasmodic affections of lungs and diaphragm.
7. HEPAR SULPH – sore, ulcerated nose with catarrhal troubles. Sneezing every time he goes into a cold, dry wing, with running from nose, later thick, offensive, discharge. Stopped up easily. Quinsy , with impending suppuration. Hawking up of mucus. Loses voice and coughs when exposed to dry cold wind. Cough troublesome when walking. Cough excited whenever any part of the body gets cold or uncovered or from eating anything cold. Chocking cough, rattling, croaking, suffocative cough with anxious wheezing moist breathing, asthma worse in dry cold air
8. IPECAC – coryza, with stoppage of nose and nausea. Epistaxis. Dyspnoea, constant constriction in chest. Asthma. Continued sneezing. Wheezing cough, cough incessant and violent, with every breath. Suffocative cough, whooping cough , with nose bleeds, croup, rattling cough. Heamoptysis from slightest exertion.
9. KALI BICH – snuffles of children, especially fat, chubby babies. Pain and pressure at the root of nose, with sticking pain in it. Septum ulcerated. Fetid smell. 10. JUSTICEA – highly efficacious medicine for acute catarrhal conditions of the respiratory tract (used in the beginning). Dry throat, pain during empty swallowing, tenacious mucus. Dry cough from sternal region all over chest. Hoarsness, larynx painful. Paroxysmal cough with suffocative obstruction of respiration. Cough with sneezing. Severe dyspnoea with cough. Asthamatic attaks, cannot endure a close , warm room. Whooping cough.
11. LEMNA MINORA – a catarrhal remedy. Acts especially upon the nostrils. Nasal polypi, swollen turbinates. Atrophic rhinitis. Asthma from nasal obstruction, wore in wet weather. Laparoscopy for Non-palpable Testicle
Rationale for the Procedure and/or General Comments
Laparoscopy has been used since 1976 for the evaluation of the non-palpable testis in pediatric patients. The rationale for the procedure has been to decrease the morbidity of open standard surgical exploration for the non-palpable testicle. Furthermore, therapeutic interventions such as orchiopexy and orchiectomy are also feasible using this technique.
Technique
In the operating room under general anesthesia, a second manual palpation is performed to check for testes in the inguinal canal or scrotum. If none is found, the patient is prepped and draped in the usual manner. The primary port is inserted in the periumbilical region. A 5-mm port is placed in the contralateral lower abdominal quadrant for manipulation. A second port can be used for laparoscopic clipping and division of testicular vessels where necessary for the first part of the two-part staged Fowler-Stevens orchiopexy. During this part of the procedure, the testicle is identified and its relation to the spermatic vessels and internal inguinal ring ascertained. A testicle that is normal size for the patient’s age should be salvaged, whereas a testicle that is non-viable should be removed. If no testicle is identified on laparoscopy and blind ending spermatic vessels are seen, the testicle has atrophied and the procedure is terminated. If no testicle is identified, no spermatic vessels are seen, and only the vas deferens is seen going into the inguinal canal, the laparoscopic dissection must continue higher in the retroperitoneum in search of the undescended testicle. The second stage of the procedure is usually performed approximately 6 months later through a high groin incision mobilizing the testicle into the scrotum.

Diagnostic Accuracy of the Procedure
Diagnostic laparoscopy identifies the location of a nonpalpable testis with 99-100% accuracy (level III) [1-5]. The procedure reliably demonstrates whether the testicle is present intra-abdominally or whether the vas and the vessels enter the internal inguinal ring. When laparoscopy is applied only for diagnosis, it can still prevent unnecessary abdominal explorations in 13-18% of patients (level III) [1,3]. Inguinal exploration alone may identify up to 34% of testicles and obviate laparoscopy; however, no good predictors exist III) [3]. Laparoscopy by a skilled laparoscopist enables therapeutic intervention (orchidopexy or orchiectomy), minimizes the need for open explorations, and preserves the benefits of the minimally invasive approach. Importantly, physical examination under anesthesia prior to laparoscopy may identify up to 18% of nonpalpable testicles in the groin (level III) [3]. There are little data comparing laparoscopic and open exploration.
Procedure-related Complications and Patient Outcomes
Procedure-related complications have been described to occur in 0-3.2% of patients, the most severe being a bowel injury.
Laparoscopic-assisted orchidopexy has been associated with 0-2.2% testicular atrophy and 97% success rates. This compares favorably with the one-stage Fowler-Stephens orchidopexy (with a 22% atrophy and 74% success rate) and the two-stage Fowler-Stephens orchidopexy (with a 10% atrophy and 88% success rate) (level III) [4,5]. It has been hypothesized that laparoscopic orchidopexy may decrease the rate of testicular atrophy by preserving the vascular supply as it can be performed usually in one stage.
Cost-effectiveness
There are no available data on the cost-effectiveness of the procedure.
Limitations of the Available Literature
The quality of the available literature for laparoscopy in the management of non-palpable testis is limited to level III evidence. No studies compare the open and laparoscopic approach with regard to patient morbidity, and there is inconsistency in the use of preoperative localization studies before laparoscopy. These limitations make strong recommendations difficult.
Recommendations
Patients undergoing DL for nonpalpable testis should have physical examination of the groin under anesthesia before the procedure is started as this approach will identify up to 18% of testicles and obviate the need for the procedure (grade A). Diagnostic laparoscopy should be part of the treatment algorithm of patients with nonpalpable testis as it is likely to improve patient outcomes; however, further higher quality study is needed. (grade C).

Before communicating with the child, parents should communicate among themselves
