Hemorrhoids, or piles, is one of the most common and nagging disorders. By themselves, though hemorrhoids are rarely serious, they can be extremely troublesome. But in some instances, they may mask a more serious disorder, such as colon or rectal cancer.
Hemorrhoids are a dilated or enlarged vascular clusters which occur in and around the anus and the rectum. They may be external i.e outside the anus or internal and slip to the outside.

4.Water is essential, and is especially important in helping to prevent constipation. It is recommended that adults drink 6-8 glasses, totalling 2 litres, of water per day. It may also be helpful to limit caffeinated drinks and alcohol.
5.Regular fibre in diet , A healthy diet may consist of 30 grams of fibre per day, which may include 5 servings of fresh fruits and vegetables.
6.Increase your amount of daily physical activity.
7.Lose weight.
8.Change bowel habits. Respond to the urge to defecate and stay relaxed to avoid straining.
9. Exercise the buttocks muscles.
10. Practice good personal hygiene. Keep the anal area clean and avoid using rough toilet paper.

Ventral (incisional) hernia
Ventral hernia (also called incisional hernia) is a defect in the abdominal wall presenting as a swelling in the abdomenwhich becomes prominent on coughing or straining. It is also called incisional hernia as this arises from a previous operation scar.
Ventral herniae are commonly seen after a caserean section, abdominal hysterectomy or peurperal sterilisation. Post-operative wound infection, obesity, chronic cough predispose to this condition.
Quite often the content of this hernia will be abdominal fat apron called the omentum. Ocassionally small or large intestine can also be a part of the hernia..
Smaller herniae may cause pain and sometime the contents of the hernia may get obstructed at the neck of hernia causing severe pain and needing emergency surgery.
Most of these herniae will require surgical treatment in the form of a mesh repair using a sophisticated synthetic mesh. These days majority of the ventral hernaie are operated by laparoscopic method with minimal pain and good outcomes.
These hernia have a tendency to recur needing re-operation. Certain good surgical principles if followed can prevent a recurrence.
Hence any unusal swelling in the abdominal wall should always be taken seriously and treated

Acid injury of the food pipe
Consumption of bathroom cleaning acid is one of the most common methods of attempting suicide especially in young girls and most often this does not kill but leaves the patient in a very miserable state of affairs impairing their ability to swallow and subjecting them to multiple hospital visits, endoscopic treatment and quite often surgery. And not all of them will be cured by any of the medical and surgical measures.

Being easily available and cheap , bathroom cleaning acids are one of the commonest sources ingested for suicidal purposes. Accidental ingestion by toddlers and children is not uncommon.

These victims present with severe chest and abdominal pain, vomiting and burning sensation in the mouth which usually settles down in 2 - 3 days. They will then be able to swallow liquids and later solids. The real effect of acid injury to the food pipe will show after about 40-50 days after the incident when swallowing of liquids will also not be possible needing hospitalisation and treatment

The sordid saga will start from then on. Barium x-rays, endoscopic dilatation and often major abdominal surgery will be required to enable the patient to swallow. The involvement of mouth, voice box and upper food pipe makes surgery very difficult with poor outcomes. At times we resort ot silencing the voice box with a permanent tracheostomy to enable eating.

There will be huge drain on the family both emotionally and financially and the worst bit is that no one will have any sympathy to the patient.

Corrosive acid ingestion will not kill a person but will cause considerable damage. One has to be very careful with bathroom acids. these shouldnt be left in the bathroom within the reach of children.

Acupuncture triggers adenosine and ATP metabolites release
I came across this authentic research explanation on the mechanism how acupuncture works. I am sharing this in this forum.

Acupuncture is a procedure in which fine needles are inserted into an individual at discrete points and then manipulated, with the intent of relieving pain. Since its development in China around 2,000 B.C., acupuncture has become worldwide in its practice1. Although Western medicine has treated acupuncture with considerable skepticism2, a broader worldwide population has granted it acceptance. For instance, the World Health Organization endorses acupuncture for at least two dozen conditions3 and the US National Institutes of Health issued a consensus statement proposing acupuncture as a therapeutic intervention for complementary medicine. Perhaps most tellingly, the U.S. Internal Revenue Service approved acupuncture as a deductible medical expense in 1973.

Although the analgesic effect of acupuncture is well documented, little is understood about its biological basis. Insertion of the acupuncture needles in itself is not sufficient to relieve pain4. An acupuncture session typically lasts for 30 min, during which the needles are intermittently rotated, electrically stimulated or, in some cases, heated. The pain threshold is reported to slowly increase and to outlast the treatment4. The primary mechanism implicated in the anti-nociceptive effect of acupuncture involves release of opioid peptides in the CNS in response to the long-lasting activation of ascending sensory tracks during the intermittent stimulation4–6. However, a centrally acting agent cannot explain why acupuncture is conventionally applied in close proximity to the locus of pain and why the analgesic effects of acupuncture are restricted to the ipsilateral side7,8.

RESULTS
Acupuncture triggers adenosine and ATP metabolites release

ATP is released in response to either mechanical and electrical stimulation or heat. Once released, ATP acts as a transmitter that binds to purinergic receptors, including P2X and P2Y receptors9,10. ATP cannot be transported back into the cell but is rapidly degraded to adenosine by several ectonucleotidases before re-uptake10. Thus, adenosine acts as an analgesic agent that suppresses pain through Gi-coupled A1-adenosine receptors11–13. To determine whether adenosine is involved in the anti-nociceptive effects of acupuncture, we first asked whether the extracellular concentration of adenosine increases during acupuncture.

We collected samples of interstitial fluid by a microdialysis probe implanted in the tibialis anterior muscle/subcutis of adult mice at a distance of 0.4–0.6 mm from the ‘Zusanli point’, which is located 3–4 mm
below and 1–2 mm lateral for the midline of the knee4. Adenine nucleotides and adenosine were quantified using high-performance liquid chromatography (HPLC) with ultraviolet absorbance before, during and after acupuncture (Fig. 1a)14,15. At baseline, the concentrations of ATP, ADP, AMP and adenosine were in the low nanomolar range (Fig. 1b), consistent with previous reports16,17. Acupuncture applied by gentle manual rotation of the acupuncture needle every 5 min for a total of 30 min sharply increased the extracellular concentrations of all purines detected (Fig. 1b).

Adenosine concentration increased ~24-fold (253.5 ± 81.1 nM from a baseline of 10.6 ± 6.7 nM) during the 30-min acupuncture session (Fig. 1c). The extracellular concentration of ATP returned to baseline after acupuncture, whereas adenosine, AMP and ADP remained significantly elevated (adenosine and AMP, P < 0.01; ADP, P < 0.05, paired t test compared to 0 min) at 60 min (Fig. 1c). Notably, previous studies have shown that deep brain stimulation is also associated with a severalfold increase in extracellular ATP and adenosine. Similar to electroacupuncture and transcutaneous electrical nerve stimulation, deep brain stimulation delivers electrical stimulation that triggers an increase in extracellular adenosine concentration18.

Ayurvedic approach to treating stress
To be effective in treating stress, it’s important to be specific. With the Ayurvedic approach, we always try to target the etiological (causal) factors and then bring that area into balance. But we are careful not to create imbalance in other areas that may produce negative side effects.

Vata, Pitta and Kapha are the three psycho-physiological Ayurvedic principles (called doshas) that govern all the activities of the mind and body. A person enjoys perfect health if these principles are in perfect balance.
Each person’s constitution tends to be dominated by either one or two of these three doshas and very rarely by all three doshas. But whatever one’s constitution is, when a person goes out of balance, Vata destabilizes most easily. Vata is the dosha mainly affected by stress. We can explain this because Vata governs all movements in the body and the nervous system. This includes the in-and-out flow of the breath, the action of our limbs, the circulation of subtle energy in our organism, every current running through a nerve and the mind’s ceaseless flow of thoughts, words, and images. If there is too much movement in body or mind, which happens if stressed, the balance of Vata will be affected and imbalance created.

The nervous system becomes overstimulated with stress, creating an imbalance. Therefore Vata goes easily out of balance. Since dry, rough, airy, fast-moving, and irregularity are the core qualities of Vata, the Ayurvedic approach is to prescribe treatments, activities and foods that provide the opposite qualities: oiliness, grounding, slowness, heaviness, consistency and regularity.

There are three different manifestations of day-to-day stress from the perspective of Ayurveda - mental, emotional, and physical. Each requires different approaches and therapies.
Mental Stress
Mental stress, according to Ayurveda, is caused by an overuse or misuse of the mind. For instance, if you perform intense mental work many hours a day, or if you work long hours on the computer, it can cause an imbalance in Prana Vata, the mind-body operator concerned with brain activity, energy and the mind. The first symptom of Prana Vata imbalance is losing the ability to handle day-to-day stress. As the person becomes more stressed, it impacts mental functions such as dhi, dhriti, and smriti-acquisition, retention, and recall. The person’s mind becomes hyperactive, yet the person loses the ability to make clear decisions, to think positively, to feel enthusiastic, and even to fall asleep at night.
Therapies for mental stress
To address day-to-day mental stress, it is important to begin by managing mental activity. Secondly, you can take measures to pacify Prana Vata, for example, by:
• Favouring Vata-balancing foods, such as sweet, sour, and salty tastes, warm, nourishing, liquid and heavier foods
• Favouring warm milk and other light dairy products
• Performing a full-body warm oil self-massage everyday in the morning before the shower. This is called abhyanga in Ayurveda
• Doing breathing exercises (Pranayama) in the morning and evening
• Having a regular daily routine
• Eating meals at regular times and taking your time when you eat, avoiding eating on the run
• It is important to get plenty of rest, and if you are having trouble falling sleep, avoid stimulants like caffeine and sip on herbal tea instead. Relaxing aromatherapy and meditation can help calm the mind.
Emotional stress
Emotional stress can be caused by a problem in a relationship, the loss of a relative, or any situation that might hurt the heart. Emotional stress shows up as irritability, depression, and emotional instability. It affects sleep in a different way than mental stress - it can cause you to wake up in the night and not be able to go back to sleep.

Emotional stress disturbs Sadhaka Pitta, the mind-body operator concerned with the emotions and functioning of the heart.
Therapies for emotional stress
To balance emotional stress, you need to favour Pitta-pacifying foods and routine, such as:
• Favouring Pitta-pacifying foods such as the sweet, bitter and astringent tastes.
• Eating lots of sweet juicy fruits
• Drinking a cup of warm milk with cooling rose petal preserve or a pinch of fennel and cardamom before bed
• Cooking with Pitta-pacifying spices such as cardamom, coriander, cilantro (fresh coriander), fennel, and mint
• Going to bed before 10:00 p.m
• Taking regular breaks and time out avoiding overworking or trying to be an over achiever and Mr or Mrs Perfect. Moderation, don’t overwork.
• Keep cool. Avoid hot temperatures and food. Favour cool foods and drinks. Sweet, bitter and astringent tastes. Cool means at room temperature but not fridge or freezer temperature.
• Reduce spicy, pungent, sour, salty tastes and hot, oily foods.
• Allow for leisure time.
• Regular mealtimes, especially lunch at noon.
• Abhyanga (Ayurvedic oil massage) with cooling oil such as coconut.

All Pitta products: drink Pitta tea and season meals with Pitta churna (powder), practice cooling aromatherapy with Pitta aroma oil e.g. mint, sandalwood.
Physical stress
Physical stress is caused by misuse or overuse of the body, such as exercising too much or working for extended periods at a job that is physically taxing. This can cause a person to experience physical fatigue, along with mental fogginess, difficulty in concentrating, and dullness of the mind.

Excessive physical strain causes three sub-doshas to go out of balance: Shleshaka Kapha, the subdosha concerned with lubrication of the joints and moisture balance in the skin, Vyana Vata, which governs the circulation, nerve impulses and the sense of touch, and Tarpaka Kapha, which governs the neuro humors.
Another reason for physical stress can be too little exercise, which results in a sluggish digestion and the formation of ama, the digestive impurities that clog the channels. In either type of physical fatigue, the process of regenerating cells slows down, and thus the cells themselves become physically tired.
Therapies for physical stress
The solution is to balance Vata and to support Kapha to make the body more stable and nurturing, for example, by:
• Getting adequate rest and moderate exercise
• Taking breaks and time out
• Following a Vata-Kapha pacifying diet (like warm meals, not to light nor to heavy or oily, see your Ayurvedic practitioner for more particular information)
• Eating regularly and avoiding to eat on the run
• Performing the full-body warm oil self massage everyday

Yoga for better Sleep
Sleep means total relaxation of body and mind, no activity of mind, no movement in mind. It is the state of unconsciousness as for as outwards moving consciousness is concerned. Mind does not function in real sleep. If dream is there, it is not sleep.
Sleep is very important support to health. Sleep holds a unique value for Health and can be even considered as one of the boons which God has given to Human beings. Natural sleep gives complete rest to the body and mind and provides psychic energy and enthusiasm. But if we do not know how to take natural sleep, it may not be much helpful in giving rest. To keep anyone awake without allowing him to sleep is one of the cruelest punishments that can be given to anybody. If sleep is taken away from human beings, the entire world will be converted into a madhouse.
A tired and spent person gets relief and is able to retrieve his liveliness and energy only after taking sound sleep. Sleep is a law of nature which is applicable not only to human beings but also to the botanical world. Our physical and mental health depends a great deal on sleep. In our country, the rule of 'early to bed and early to rise' has been followed traditionally. However, this practice has been very adversely affected by the fast pace of urbanization and industrialization. In this context, this is also relevant to remember that sleep of 6 to 8 hours at any point of time during the 24 hours of a day is not sufficient for health. This sleep, in order to be beneficial to health, should be taken only at the appointed time of nature i.e. during night.
Now a day insomnia and sleeplessness is common problem amongst people due to alteration in working pattern, tension, anxiety, uneven life style etc. We are not taking this fact seriously .This will lead to many incurable and dangerous life threatening problems. We are destroying our body and health. We are so busy that we are not spending a little time for our health.
Yoga and Pranayama is an important measure to reduce work place negativity, tension, anxiety and so many other problems. Regular practice of pranayam and yoga cures insomnia and sleeplessness and gives better health. If every person follows this regime, this will eradicate diseases from world.
• Bhastrika pranayam 3 to 5 minute
• Kapalbhati pranayam 3 to 5 minute
• Anulom Vilom pranayam 5 to 10 minutes
• Bhramari pranayam 5 to 10 times
• Udgith Pranayam 5 to 10 times
• Ujjai Pranayam 5 to 10 times
Besides this Shavasan and Yognidra is very much useful for sound sleep. These practices are able to cure all types of ailments. Listening to light music like mantras before sleep is beneficial in insomnia and sleeplessness.
