PreconditionsХ Diagnosis compatible with brain stem death
Х Presence of irreversible structural brain damage
Х Presence of apnoeic coma
Exclusions
Х Therapeutic drug effects (sedatives, hypnotics, muscle relaxants)
Х Hypothermia (Temp >35░C)
Х Metabolic abnormalities
Х Endocrine abnormalities
Х Intoxication
Clinical tests
Х Confirmation of absent brain stem reflexes
Х Confirmation of persistent apnoea
Х Clinical tests should be performed by two experienced practitioners
Х At least one should be a consultant
Х Neither should be part of the transplant team
Х Should be performed on two separate occasions
Х There is no necessary prescribed time interval between the tests
Clinical tests for absent brain stem reflexes
Х No pupillary response to light
Х Absent corneal reflex
Х No motor response within cranial nerve distribution
Х Absent gag reflex
Х Absent cough reflex
Х Absent vestibulo-ocular reflex
Test for confirmation of persistent apnoea
Х Preoxygenation with 100% oxygen for 10 minutes
Х Allow PaCO2 to rise above 5.0 kPa before test
Х Disconnect from ventilator
Х Maintain adequate oxygenation during test
Х Allow PaCO2 to climb above 6.65 kPa
Х Confirm no spontaneous respiration
Х Reconnect ventilator
